1067436396 2013-03-26 Follow up to 1361476913. The cracked poly tank has been half emptied of fluids and is no longer leaking.
1234550762 2014-01-28 Follow up to 1361476913. The cracked poly tank has filled with fluids again and is leaking down the side and onto the ground. Field test strips used to measure the Chloride content of the water and it was found to have 36 ppm Chlorides. Still no production recorded.
-1255577667 2014-04-11 I spoke with Mike Bendimen and he stated that he plans on completely removing the tank battery from this well. He will plumb it together with the Flanders well to the new tank battery. He will have the contamination cleaned up within the next week. Once he finishes the cleanup he will begin working on getting the well back into production.
1329159146 2012-02-13 Compliance notice follow up. The original cracked tank is still on site and is roughly two-thirds full. A crack has developed and split down the side of the tank, the fluid level is even with the bottom of the crack. A secondary steel tank has been placed at the tank battery. This secondary tank has a hole in the side and is unable to prevent the escape of oil and gas.

1361476913 2013-02-19 A 2100 gallon poly tank is along the road and is roughly two-thirds full. A crack has developed and split down the side of the tank, the fluid level is even with the bottom of the crack. There was no dike around the tank and the tank was situated roughly 15 feet from the county road and in a way so that a potential spill may affect the traveled portion of the road. A small area of contamination is visible below the crack. All contaminated soils shall be removed and disposed of properly. The affected area shall be seeded and mulched and steps shall be taken to encourage vegetative cover. Cracked tank shall be repaired or replaced. Tank(s) shall be placed inside a dike with a capacity enough to hold 110% of the largest vessel and meeting all current spacing requirements.

There is a two inch steel line plumbed into the plastic 100 Barrel tank in the new dike. The old cracked 1100 gallon poly tank with the contamination has been removed.

-1990821468 2011-08-05 Well identification was not legible, no production records since 2008, plastic production tank had folded in on itself and cracked, tank only 15 feet from traveled portion of roadway. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tanks as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. Well shall be produced, plugged, or placed in temporary inactive status. Tank shall be replaced or repaired. Oil production tanks shall be set a minimum of fifty (50) feet from the traveled portion of a public road and a minimum of one hundred (100) feet from existing inhabited structures and a minimum of three (3) feet between tanks and a minimum of fifty (50) feet from any well.
-2147168757 2014-04-10 Compliance Notice Follow-up 1361476913: There has been no resolution to any of the issues. The damaged tank is again 3/4 full of fluid up to the top of the crack and is leaking down the side. There is visible contamination on the ground. The identification is illegible at this time and posted on the side of tank. I spoke with the landowner Tom and he stated the well has not produced for years. I called Luke Mallett and Mike Bendimun and left messages regarding the outstanding issues.

-787017 2013-01-31 Follow up inspection to -1990821468. The 2100 gallon plastic tank has not been removed and is less than 10' from the road. The side is still cracked and a small amount of contamination has leaked out of the crack. The new tank has been installed but ahs not been connected.
