| Inspection this day found no one at well.  Pump running, reclamation is being worked on and needs to be finished, not done yet, due to the weather, I believe.  Pressure chart at well, good ID's, water coming thru pipeline from 2+ miles away - other well at this facility on the river where tanks, pump are located.

| I met today with Tim Leftwich & Doug Aden, OGS and Glenda Besana-Ostman, DNR- UIC,  to show them possible good sites for further seismic monitors.  We discussed the requirements needed for a good site and I showed them one outside Newport, located between 2 soon-to-be approved new disposal wells and they installed one of the units this day.  Also showed them several other sites (one of which is near this well) to choose from for monitors they will bring in following weeks.

| Rig crew on location, getting ready to leave.  Waiting on orders, was told a parafin block in 4.5" casing, rental tubing on location and will go in with small bit and drill out blockage - well flowing this day so are not going to work on it.  Water getting around this blockage so not solid, my idea to go in with solvent, let set and see if that doesn't take care of it.  Hopefully, can run packer 7/21/2008.

| Inspection this day found well hooked up to computer - no longer have a way to put my gauge on.  Chart shows annulus and tubing pressure, also is being monitored thru computer at pump house.  All looks good - pumphouse is spotless!  Talked to T. Tomastic and he said fine if I use their computer readings for pressure.

| On Friday, 9/26, service rig set up with Baker man and unhooked packer.  Ran dye down tubing to try and determine where leaking - dye circulated with the conclusion that tubing had the leak, not the packer.  Tubing was re-connected to packer and both were pulled, finishing up on 9/29/08.  As tubing was pulled, found all the joints to be lose enough to leak, surprising as power tongs were used to tighten pipe.  Also, several plastic collars used with the coated pipe were found in the tubing, some crushed, and coating on the pipe appears to be damaged.  Also marks on packer indicating some rough treatment.  New coated pipe was ordered. 10/2 Packer was run back in hole along with new coated pipe and collars.  Tally on tubing that had come out of hole was off about 25' - maybe measured wrong or missed a joint.  Baker man present to run packer back in and observe.  10/4 Pulled tubing and packer, 10/5- Ran tubing, packer back in, this time tightening joints over recommended limits, 10/6- Set packer, closed valve, tested tubing and annulus, both holding, circulated inhibitor, tested tubing again and started injecting.

| Inspection found service rig on location - were supposed to run packer on tubing and pressure test but having trouble with the well - can't get down the casing with tools, black, sticky substance coming out of well and took sample to be analyzed.  Analysis back today says it is parafin - where coming from?  Choices are dirty frac tanks or from formation, I would say.  Waiting on plan to remove blockage and run tubing.

Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
-1009930694 2010-08-23 Inspection this day found well in production, pump running. Pressure on tubing is 1820#, 0# on annulus. Good ID's, all looks good. Well is being charted.
-1025084211 2009-08-31 Inspection this day found pumps running, well taking water. All looks good.
-1031027108 2009-07-23 Inspection this day found location has been reclaimed, completely finished and grass growing, no debris. Injection well.
1053242287 2010-11-22 Inspection this day found well hooked up to computer - no longer have a way to put my gauge on. Chart shows annulus and tubing pressure, also is being monitored thru computer at pump house. All looks good - pumphouse is spotless! Talked to T. Tomastic and he said fine if I use their computer readings for pressure.
1222789100 2008-10-06 On Friday, 9/26, service rig set up with Baker man and unhooked packer.
1312378559 2011-08-02 Inspection this day found no problems, pumps clean, housed, running
1322067122 2011-11-21 Inspection this day found no change in well or facility. Facility is clean, the integrity of pipe good, proper identification is posted, operational flowmeter and pressure gauge at well. Pressure readings were taken off computer at office.
-1332328564 2008-11-19 Inspection this day found no one at well. Pump running, reclamation is being worked on and needs to be finished, not done yet, due to the weather, I believe. Pressure chart at well, good ID's, water coming thru pipeline from 2+ miles away - other well at this facility on the river where tanks, pump are located.
1386965570 2013-12-13 Inspection revealed that injection pump was in operation and it was noted that injection pump has been slowed down at request from Central Office.
1388100657 2013-12-23 On site with company personnel to test annulus of tubing they pressured up too 300# and we monitored pressure with a small leak detected within tubing head, bleed pressure off and tighten head and retested and it still leaked a little so decision was made to order new packing for tubing head and once packing comes in we will retest well.
-1430863003 2012-11-20 Inspection revealed that injection pump was in operation during my inspection and well site was in satisfactory condition this inspection.
1447494134 2010-02-24 Inspection this day found no violations, pumps running and this well taking water at 1675# on tubing, 0 on annulus.
1453617165 2008-07-31 Inspection this day found crew doing flow testing. Well injecting at 56 bbl/hour at 1200# - water being trucked into tanks on location - short piece of line connecting well to tanks. Pipeline not completed yet.
1475059360 2008-07-16 Called out today as they are ready to pump saltwater into the temporary pit at the oil gathering facility. Heavy plastic liner has been hot-welded together to prevent leaks - water will start going in 7/17/2009 - they will have 120 days.
-1477582439 2012-10-30 I met today with Tim Leftwich & Doug Aden, OGS and Glenda Besana-Ostman, DNR- UIC, to show them possible good sites for further seismic monitors. We discussed the requirements needed for a good site and I showed them one outside Newport, located between 2 soon-to-be approved new disposal wells and they installed one of the units this day. Also showed them several other sites (one of which is near this well) to choose from for monitors they will bring in following weeks.
-1480015238 2013-01-16 Inspection on this day revealed that injection pumps were in operation and found pump house clean and neat and facility appears to be in satisfactory condition this inspection.
-1530501340 2010-05-24 Inspection this day found pump running, pressure on well. Good ID's, no violations.
-1565442962 2008-07-15 Rig crew on location, getting ready to leave. Waiting on orders, was told a parafin block in 4.5 casing, rental tubing on location and will go in with small bit and drill out blockage - well flowing this day so are not going to work on it. Water getting around this blockage so not solid, my idea to go in with solvent, let set and see if that doesn't take care of it. Hopefully, can run packer 7/21/2008.
1621469158 2008-09-08 Met with Bruce Phipps of OOG regarding tubing leak. Well showed 600# on tubing, 600# on casing, and had been shut down since Friday (9/5/08). Talked to Tom Tomastik to get his opinion, then shut well down until tubing leak fixed. Service rig could have been on location today but Baker who operates packer could not get there for 2 weeks, Tom Roberts reports.
1625873439 2012-12-12 Inspection revealed that injection pump was in operation today and pump house was clean and tidy this inspection.

Note: Lease road to wellhead is in need of grade work preformed.

1701304756 2008-08-18 First inspection since pipeline has been completed and are injecting water from their facility along the river, not tanks on location which they were temporarily hauling into. Tubing pressure 1400#, annulus pressure 800#. Still need ID's. Location has not been reclaimed. Operator reports tubing or packer is leaking - they think tubing and have rig scheduled for 8/25/08 to pull and find problem.
1776054022 2013-04-30 Inspection revealed that injection pump was in operation during my inspection.

Note: 2 nipple on injection line is showing signs of needed replacement and also high pressure hose going to meter box is showing signs of fatigue.

-182746821 2008-08-21 Inspection this day due to complaint by landowner 1/4-1/2 mile south on Long Run Rd. - complaining that water well has gone bad. Well inspection found tubing pressure 1200#, annulus was 200#, well producing. Jeremy Seevers called to say well contaminated with brine
-1893203788 2008-07-24 Inspection this day found service rig circulating on hole thru the bit and 2 7/8 casing. Got back down to bottom, will come out and start running coated pipe with packer on Monday morning. Well perf'd at 6978-7134' - blockage had been at 6405' - they believed it to be paraffin.
