Pad and impoundment were constructed. BMPs were installed as per plans. Slopes were seed and mulched and had obtained vegetative growth. Rigs was being trucked in. Secondary containtment was being installelled. Site plan, E&S plan, and site specific PPC plan were all in mailbox. No sign was currently installed. An email was sent to Randy Reimold requesting one be posted. No violations noted at this time.

2029349 2011-12-23 No Violations Noted Site inspection performed as a follow-up to spill of synthetic drilling mud to the containement on the well pad. The spill had been cleaned up as of the time of the inspection. Erosion controls had been implemented properly and there was no accelerated erosion or sedimentation in relation to the access road or well-site. Disturbed area had been temporarily stabilized. Well stimulation impoundment located onsite. Impoundment conatined fluids and ad more than the required two feet of lined freeboard.

2034891 2012-01-17 No Violations Noted H&P Rig 287 was running at time of inspection. Fred Webb was the company man. Mr. Webb and DEP walked the site. Two french drains were found leading from the pad into a drainage ditch. Both drains had a small amount of suds in their drainage. Mr. Webb closed the valve on the main drainage route leading from the protective diking effectively stopping the discharge of stormwater from the site.

Inspection of rig found chemical totes next to the compressors. These totes were placed on the edge of the secondary containment with their valves hanging over the non-containment part of the pad. Suds were visible beneath the valve of the tote containing soap. This is the most probable source of the contamination. Other areas of secondary containment were comprimised where trucks had driven over edges. Sample 0629735 was taken in the drainage ditch on the South corner of pad.

2037281 2012-01-24 No Violations Noted Site visit was follow-up to 1-17-12 inspection with violations. Fred Webb (WPX Company Man) was on-site.

No soapy discharge was visible. French drains were tied together and flowed via gravity to the impoundment. Totes were moved on containment. Various areas of the secondary containment were comprimised (ie berm was knocked down in various areas). H&P representative said those were to be corrected soon. A small amount of what appeared diesel fuel appeared to be spilled on the pad. The stone from that area was immediately excavated and played in a roll-off box for disposal. Site plans were inspected at entrance to site. There is no design for the small depression made to place roll of box for cement returns. This addition might constitute a major modification. The underdrains are also not marked on the plans. Preliminary sample (0629735) results do show surfactants in the water.

2232965 2014-01-02 No Violations Noted At the time of inspection the Department notes that the well appears to be drilled but not equipped for production. The Department received stimulation notice and the pressure test notification for this well on 12/31/2013. The Erosion and Sedimentation Plan was onsite and appeared to be implemented as designed. No sign of accelerated erosion or sedimentation observed in relation to the well pad or access road during the inspection. The impoundment associated with the well also appears to be in sound condition and had the required 2' of freeboard. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation event to ensure the stability of the site in maintained.
