| Weather cloudy and cold, inspected with WQS Ben Bahner. This was a preoperational inspection conducted while the pad was still under construction. There were a number of wetlands in the vicinity, and there was a wetland waiver on site. Perimeter E&S controls were in place and conformed with the location and dimentions on the ESCGP plans. At culvert CDC-4, where the access road bends around the base of the pad the rock apron outlet terminates onto a wetland finger. Given its location we recommended modifying the outlet to employ larger size rock and to decrease the overall side. The outlet was modified. There were no other issues at this location.There were no wells on the pad. Inspections for the yet undrilled wells have been logged in DEP's eFACTS database as per guidance in the DEP Draft Oil and Gas Surface Activities Inspection Manual (version June 2012).

| Chesapeake was in the process of breaking down secondary containment used for recent hydraulic fracturing operations.  I met with Jeff Parkhurst of CHK.  I noticed a small area of a substance green in color spilled to the containment liner near where the brine tanks were stored and what appeared to be a few drips or drops of a substance spilled to the pad surface at N- 41.58720°; W-076.21226°.  I pointed out the areas where the substances were released to Mr. Parkhurst.  He stated he would have the areas cleaned up as soon as possible.

The 9x5 and 13x9 vents were shut in and had 0 psi at time of inspection. The 20x13 vent was open to atmosphere and 0% combustible gas was detected at time of inspection. | I arrived at the site at 10:43. The weather conditions are mostly cloudy skies with light winds, and temperatures in low to mid 30's. Patterson Drilling Rig # 572 is on the pad drilling the Alvarez 101H well.  I met with Glenn Wolf, site company man, and consultant of CHK, Isabel Leon, Drilling Engineer of CHK, and Kris Gonzalez, Drilling Superintendent of CHK.  The current drilling depth is (15,786) feet.  CHK submitted a Drilling resumption After Break of 30 days or more notification to the Department on 11/27/18.  The comments in the notification indicated CHK would resume drilling on the Alvarez 101H well at 2 AM on 11/29/18.

Performing inspection due to notification from Chesapeake of a communication event between this well and the Adams well to the east while they were fracturing. Well was squeezed at 7250' to try to stop flow in the annulus. They were intending to circulate good cement from 7250' to and estimated 1500' to bring cement up inside the intermediate casing. Halliburton pumped 258 bbls of 15 ppg Expandacem cement through the perforations. Reviewed the cement job log and no discrepancies were noted. They ran a bond log on Sunday and determined that they did not get good cement behind the casing. Well and its annular spaces will be monitored. Well was covered with rig equipment at time of inspection. Rig was drilling out plugs on the other wells on location. Rig performed a perforate and squeeze operation on the well to achieve better zonal isolation. They perforated at 7475' and had a retainer set at 7310'. They pumped 20 barrels of cement. 15 barrels were injected into the perforations and 5 were left in the pipe on top of the retainer. They drilled out the cement and retainer  and then performed a bond log. Results of the bond log were sent to DEP via email to be reviewed. No violations were noted at time of inspection.

This was a preoperational inspection conducted while the pad was still under construction. There were a number of wetlands in the vicinity, and there was a wetland waiver on site. Perimeter E&S controls were in place and conformed with the location and dimentions on the ESCGP plans. At culvert CDC-4, where the access road bends around the base of the pad the rock apron outlet terminates onto a wetland finger. Given its location we recommended modifying the outlet to employ larger size rock and to decrease the overall side. The outlet was modified. There were no other issues at this location.There were no wells on the pad. Inspections for the yet undrilled wells have been logged in DEP's eFACTS database as per guidance in the DEP Draft Oil and Gas Surface Activities Inspection Manual (version June 2012).

2357521 Primary Facility 02/24/2015 Routine/Complete Inspection No Violations Noted Chesapeake was in the process of breaking down secondary containment used for recent hydraulic fracturing operations. I met with Jeff Parkhurst of CHK. I noticed a small area of a substance green in color spilled to the containment liner near where the brine tanks were stored and what appeared to be a few drips or drops of a substance spilled to the pad surface at N- 41.58720°; W-076.21226°. I pointed out the areas where the substances were released to Mr. Parkhurst. He stated he would have the areas cleaned up as soon as possible.

2804381 Primary Facility 2018-11-14 Routine/Complete Inspection No Violations Noted Weather was 32° and sunny at time of inspection. Met with Chesapeake representative Joe Button on location. Well was shut in at time of inspection. Surface discharge manifold equipment was removed in preparation for the drilling rig to move on location and drill the 101H and 103H. Well had 3 annulus vent plumbed to surface. The 9x5 and 13x9 vents were shut in and had 0 psi at time of inspection. The 20x13 vent was open to atmosphere and 0% combustible gas was detected at time of inspection. No violations were noted at tome of inspection.
2820258 Primary Facility 2018-12-12 Routine/Complete Inspection No Violations Noted I arrived at the site at 10:43. The weather conditions are mostly cloudy skies with light winds, and temperatures in low to mid 30's. Patterson Drilling Rig # 572 is on the pad drilling the Alvarez 101H well. I met with Glenn Wolf, site company man, and consultant of CHK, Isabel Leon, Drilling Engineer of CHK, and Kris Gonzalez, Drilling Superintendent of CHK. The current drilling depth is (15,786) feet. CHK submitted a Drilling resumption After Break of 30 days or more notification to the Department on 11/27/18. The comments in the notification indicated CHK would resume drilling on the Alvarez 101H well at 2 AM on 11/29/18.

CHK provided me with the following details pertaining to site activities and operations. "We are on the Alvarez 101H well. We are about (1,000) feet from total depth (TD) of drilling this well which will be (16,734) feet. The production casing will be cemented this Friday or Saturday. The Alvarez 103H is the next well to be drilled to TD. We are in the lower marcellus format

2843735 Primary Facility 2019-02-25 Routine/Complete Inspection No Violations Noted Weather was 29° and partly cloudy at time of inspection. There were wind gusts up to 30 mph. Met with Chesapeake representative Cody Blanton on location. Performing inspection due to notification from Chesapeake of a communication event between this well and the Adams well to the east while they were fracturing. Well was squeezed at 7250' to try to stop flow in the annulus. They were intending to circulate good cement from 7250' to and estimated 1500' to bring cement up inside the intermediate casing. Halliburton pumped 258 bbls of 15 ppg Expandacem cement through the perforations. Reviewed the cement job log and no discrepancies were noted. They ran a bond log on Sunday and determined that they did not get good cement behind the casing. Well and its annular spaces will be monitored. Well was covered with rig equipment at time of inspection. Rig was drilling out plugs on the other wells on location. Chesapeake intends to evaluate the bond log and re-squeeze the well at a different interval to obtain more des
2851238 Primary Facility 2019-03-11 Routine/Complete Inspection No Violations Noted Weather was 36° and sunny at time of inspection. Met with Chesapeake representative Cody Blanton on location. Rig performed a perforate and squeeze operation on the well to achieve better zonal isolation. They perforated at 7475' and had a retainer set at 7310'. They pumped 20 barrels of cement. 15 barrels were injected into the perforations and 5 were left in the pipe on top of the retainer. They drilled out the cement and retainer and then performed a bond log. Results of the bond log were sent to DEP via email to be reviewed. No violations were noted at time of inspection.
