| Routine inspection of Samson Exploration's C5 well pad on September 19, 2017. No one was onsite during my inspection. This vertical well has recently been plugged. At this point no plugging certificate has been issued by the Department for well 023-20122. Tanks and production equipment have been removed from the site. There is no ESCGP associated with this well permit because it falls under the 5 acre threshold. E&S controls have been put in place for final restoration. The site is to be restored back to approximate original contour along with the access road leading to the well pad.

| Routine inspection of Samson Exploration's B Well Pad on November 21, 2017. This site is not covered an ESCGP; the total project area is approximately 2.1 acres. There is one vertical Marcellus well that has been recently plugged. The plugged and abandoned well is identified will a tagged well monument. The site has been restored back to approximate original contour. Runoff from the restored pad has been directed towards the NE edge of the site. The culvert pipe has been removed and the natural drainage-way has been restored. Germination of grasses has started. E&S BMP compost sock remains in place. I recommend installing another 18" or larger compost sock down gradient of the restored culvert pipe area (photo 5). In addition, the roll off dumpster will need to be removed from the site.

| Department representative Sue Herzing conducted a routine site inspection at Samson Exploration's B Well Pad on June 22, 2018. This site is not covered by an ESCGP; the total project area is approximately 2.1 acres. There is one vertical Marcellus well (023-20122) that was plugged in the Fall 2017. The P&A well is identified with a tagged well monument. The site has been restored back to approximate original contour. The culvert pipe has been removed and the natural drainage-way has been restored. During the inspection I observed a spring located at the top end of the restored well pad causing erosion issues and little vegetative growth (see photos). I recommend addressing the erosion issues and re-seeding where needed. Compost socks remain in place.  A post plugging well site restoration report will need to be submitted for this plugged well.

| On August 1, 2018 Department representative Sue Herzing conducted a restoration site inspection at Samson Exploration's Well Pad B with Steve Trujilo of Samson and Joe Kunst of ARM Group. This site is not covered under an ESCGP; the total project area is approximately 2.1 acres. There is one vertical Marcellus well (023-20122) that was plugged in the Fall 2017. The P&A well is identified with a tagged well monument. The site has been restored back to approximate original contour. The culvert pipe has been removed and the natural drainage-way has been restored. During a previous site inspection in June 2018, I observed a spring located at the top end of the restored well pad causing erosion issues and little vegetative growth. Samson's contractor has addressed the erosion issue by installing an under-drain system along the upper portion of the restored well pad. The under-drain systems consists of a 3-4 ft. deep ditch with gravel placed on the bottom of it, a 4" black corrugated pipe installed, then more grav

| On October 30, 2018 Department representative Sue Herzing conducted a routine site inspection at Samson Exploration's Well Pad B with Steve Trujilo of Samson and Michael Race and Joe Kunst of ARM Group. This site is not covered under an ESCGP; the total project area is approximately 2.1 acres. There is one vertical Marcellus well (023-20122) that was plugged in the Fall 2017. The P&A well is identified with a tagged well monument. The site has been restored back to approximate original contour. A post plugging well site restoration report has been submitted and approved by the Department. During my previous inspection in August 2018,  I recommended that the operator keep E&S compost sock in place until the vegetation achieved a better root system. Since then, all compost sock has been removed. The site appears stable and exceeds 70% uniform, perennial, vegetation.

2635885 Primary Facility 2017-09-13 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of a plugged well. Rig just pulled off location recently. B Section left on well for inspector wellbore access. No gas detected, and no obvious issues were observed.
2639310 Primary Facility 2017-09-19 Routine/Complete Inspection No Violations Noted Routine inspection of Samson Exploration's C5 well pad on September 19, 2017.
2646176 Primary Facility 2017-10-06 Routine/Complete Inspection No Violations Noted Routine inspection of Samson Exploration's Pardee-Curtin B Pad on October 6, 2017. Currently the operator has placed multiple piles of soil and rock on the pad for final restoration purposes (see photos). No one was onsite working during my inspection. No E&S issues were noted during my inspection.
2663686 Primary Facility 2017-11-21 Routine/Complete Inspection No Violations Noted Routine inspection of Samson Exploration's B Well Pad on November 21, 2017.
