| At the time of inspection, site was properly signed with E&S plan. Well was drilled, not yet fractured. Filter fence was installed down gradient and functioning. Noted two pits onsite both sustained two ft of line freeboard on all sides: drill dust and minor cuttings noted outside of pit on cut slope. Spoke with Kriebel rep onsite and recommended that a diffuser be used in order to minimize/prevent splatter of dust and cuttings, which rep assured me would be addressed immediately. Access road stable, brush/grub line utilized. Culverts present, but additional rock may need to be added to outlets. No accelerated erosion or sedimentation issues were noted, but it is being recommended that ditch be installed below gradient of pad perpendicular to fresh water pit instead of haybales and filter fence in order to minimize the potential for runoff concerns during rain events.

Requested that Falcon move discharge line due to elevated amount of cuttings building up on one side of pit. Falcon was contracted. Conductor pipe (13 3/8¿) set at 42 ft (sand). Water detected at 1000¿ and 1200¿. Stratigraphy consisted of coal, ss and sh. Surface casing (8 7/8) set at 1681 ft. Well Superior present to cement surface casing. A total of 305 sks Class A cement was used. Plug down at 10:35 am. No signs of gas leak from around well and equipment.

1905451 Primary Facility 08/11/2010 Routine/Complete Inspection No Violations Noted At the time of inspection, site was properly signed with E&S plan. Well was drilled, not yet fractured. Filter fence was installed down gradient and functioning. Noted two pits onsite both sustained two ft of line freeboard on all sides: drill dust and minor cuttings noted outside of pit on cut slope. Spoke with Kriebel rep onsite and recommended that a diffuser be used in order to minimize/prevent splatter of dust and cuttings, which rep assured me would be addressed immediately. Access road stable, brush/grub line utilized. Culverts present, but additional rock may need to be added to outlets. No accelerated erosion or sedimentation issues were noted, but it is being recommended that ditch be installed below gradient of pad perpendicular to fresh water pit instead of haybales and filter fence in order to minimize the potential for runoff concerns during rain events.
1953876 Primary Facility 03/01/2011 Routine/Complete Inspection No Violations Noted At the time of inspection, well drilled and in production. Well completed 8/6/2010. Well tag present. Brine tank surrounded by dike appears sound. Site has been regraded, seeded/mulched with some vegetative growth observed. Temporary bmp¿s ( filter fence) in place. Observed minor ponding of water near filter fence, recommended operator maintain roadway culverts and outlets. Soil pile present and seeded. Access road ok. No accelerated erosion or sedimentation or IW issues noted at this time. Operator should continue to monitor all bmp¿s until final restoration of >70% successful vegetative cover is achieved.
2229306 Primary Facility 12/10/2013 Routine/Complete Inspection No Violations Noted Met with representative from Kriebel Minerals to discuss potential erosion issues. Gullies are beginning to develop below the well pad. Operator claims that he will add rock below the culvert to dissapate flow of water. Well is equipped for production, tag present. Brine tank appears to be sound and is surrounded by vegetated dike. Site is covered in snow making evaluation of vegetation growth difficult. Access road is in good condition.
