| Conducted an inspection of the RU 57 Gaylord well pad.  ID sign is in place and lists the Gaylord 3H (115-21050) and Gaylord 6H (115-20966).  The Gaylord 6H (115-20966) was not drilled and the permit expired on 8/20/2013.  A new Gaylord 6H (115-21660) permit was issued on 2/24/2014 and it replaces permit number 115-20966).  The ID sign should be updated to reflect the change.  The rock construction entrance is in place and appears to be functioning as designed.  The Gaylord 3H has been drilled and is currently in production on the pad.  No evidence of any spills or releases were observed.  E&S controls are in place and functioning.  Growth in vegetated areas of the pad is more than adequate except where more recent earth disturbance activity has taken place.

| Visited the Gaylord 3H well on March 6 1015 at around 12pm. Proper Site ID was present at edge of lease with proper permit number for the 3H and 4H which hadn't been drilled. Inspection records in mailbox showed well had been inspected by swn inspector earlier that day. Gate to pad had been broken or cut open. Production tree was in place for the 3H. No guages were present on production tree ben valve for production line out was open and sounded like it was pressurized. Cellar was completely snow covered. Cement barriers were in place around head and cellar. Well was tagged with necessary information.

| Falcon Drilling Rig #23 is currently on the location in the process of drilling the Gaylord 4H well.  SWN submitted a drilling resumption After Break of 30 days or more notification to the Department on 10-11-16.  The comments in the notice indicate that Falcon 23 will skid 20 feet from the Gaylord 2H to Gaylord 4H on 10-12-16 at 3:30 PM.  SWN submitted a casing notification to the Department on 10/11/16. The notice indicated that Falcon 23 will be setting 664' 13-3/8" H-40 48# STC casing and cementing back to surface with Schlumberger services on 10/13/16 at 6:00 AM.  I met with Tony Anthony, site company man, consultant of SWN.  The current drilling depth is (3,931) feet.  Mr. Anthony stated the following.  The intermediate casing on the 4H was set at (1,644) feet.  The 4H will be drilled to (5,748) feet which is the kickoff point.  The well will be loaded with oil based mud once it is drilled to kickoff point (100) feet above kickoff point. The 1H and 2H wells have been drilled to kickoff point.  After th

| At time of inspection, arrived on site at 11:35a.m.
 +
The cellar was full of gravel and there was no gas detected at the perimeter of the cellar.
 +
The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed.
 +
There was 590psi on the production casing and 495psi on the production line.
 +
I took a walk around the location and there were no obvious concerns noticed.

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2284462 Primary Facility 07/07/2014 Routine/Complete Inspection No Violations Noted Conducted an inspection of the RU 57 Gaylord well pad.
2349545 Primary Facility 03/06/2015 Routine/Complete Inspection No Violations Noted Visited the Gaylord 3H well on March 6 1015 at around 12pm.

2531799 Primary Facility 2016-10-17 Routine/Complete Inspection No Violations Noted Falcon Drilling Rig #23 is currently on the location in the process of drilling the Gaylord 4H well. After th
2585445 Primary Facility 2017-04-21 Routine/Complete Inspection No Violations Noted Inspection conducted on 4/18/17. Complete circuit of entire site found no violations. Observation of the stimulation operation occurring on the pad identified no spills. No violations observed.
2621158 Primary Facility 2017-08-01 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:35a.m.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 590psi on the production casing and 495psi on the production line. I took a walk around the location and there were no obvious concerns noticed. I left the site at 12:30p.m.

2660329 Primary Facility 2017-10-25 Routine/Complete Inspection No Violations Noted The Department was on site, to conduct a Routine Complete Inspection, in response to the receipt of a Well Site Restoration report. The site appeared to be reflective of the Restoration Plan therefore the Department acknowledges the Restoration.
2732533 Primary Facility 2018-05-15 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2832946 Primary Facility 2019-01-29 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was full of gravel and I was unable to check for gas at the perimeter of the cellar do too the gravel being frozen. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2855223 Primary Facility 2019-03-12 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. As the photo's show, the site is snow covered. Overall the site appears to have good stabilization. No E&S issues identified. No violations noted.
