| GDS and G.L. Parrish subcontractors of Cabot, were at the site in the process of performing post well drilling activities.  The most recently drilled well by H&P Rig #371 was the E Strauss 2H.  I observed a white powder like substance spilled to the pad surface and on containment next the oil based mud tanks in the Southwest section of the Pad at N- 41° 46' 03" ; W- 075° 43' 17".  The substance may have been either a calcium, lime or cement product to be used or used during drilling activities.  Mr. Jim Nally of Parrish, who was at the site,  contacted Cabot in regard to my discovery of the substance spilled.  I also observed small areas of dark staining on the pad surface near the well cellars where part of the containment was pulled.  The black staining appeared to be oil based mud.  There were several areas of compromised liner integrity observed.  Mr. Nally informed me that the containment will be broken down and removed from the site either on 4/14/15 or 4/16/15 and any staining discovered on the pad surface will be cleaned up as well.

| As I arrived at the site at 12:15, Cabot was in the process of setting up for the production phase of operations and conducting treatment of hydraulic fracturing flowback fluid.  Flowback operations have been completed for all wells as of yesterday 9/7/15 according to personnel on-site.  Cabot submitted an alternative waste management practice (OG-71) application on 7/28/15 to conduct treatment of flowback fluids generated and transported from the E Strauss wells through an above ground flow line to where the equipment is staged on secondary containment in the tank pad for this treatment process.  The secondary containment consists of rig mats, a layer of geotextile felt above and below a High Density Polyethylene Liner made of plastic material and corrugated pipe berm (1) foot in height encompassing the containment.  The main function of the treatment process is to remove solid material and adjust the water chemistry of the flowback fluid so it can be effectively reused during hydraulic fracturing of Cabot wells.

| On 7/26/16, the Department conducted an inspection of the COG Strauss E. Well Pad located in Harford Township, Susquehanna County.  All wells permitted for this site have been drilled, stimulated, and appear to be in production.  A relatively high number of frack tanks are being stored on the pad.  The pad is stable.  Good vegetation has been established.  Access road ditches are being maintained.  No violations noted.

The required signage was present marking the entrance to the access road. All necessary paperwork was present in the mailbox. The access road was in good condition, along with the associated PCSM BMPs. No apparent erosional issues were noted. The pad's surface was solid and stable. The cut slope and berms on the pad were well-vegetated, solid, and stable.

| Today's inspection was conducted as a routine inspection.  Jon Sassi, Department EPS was also present during the inspection.  As photo #1 shows, a slip has developed on the cut slope of the well pad.  It appears that tire track indentations were the cause of this small slip.  Please repair immediately. As photo #4 shows a small oil/grease stain was observed next to the 5H well head.  Please remediate immediately.  Photo #5 shows that recent plowing has uncovered residual felt liner.  Please remove this debris.  Other than the three items listed above, overall the site appeared stable with no violations noted.

2372133 Primary Facility 04/13/2015 Routine/Complete Inspection No Violations Noted GDS and G.L. Parrish subcontractors of Cabot, were at the site in the process of performing post well drilling activities. The most recently drilled well by H&P Rig #371 was the E Strauss 2H. I observed a white powder like substance spilled to the pad surface and on containment next the oil based mud tanks in the Southwest section of the Pad at N- 41° 46' 03" ; W- 075° 43' 17". The substance may have been either a calcium, lime or cement product to be used or used during drilling activities. Mr. Jim Nally of Parrish, who was at the site, contacted Cabot in regard to my discovery of the substance spilled. I also observed small areas of dark staining on the pad surface near the well cellars where part of the containment was pulled. The black staining appeared to be oil based mud. There were several areas of compromised liner integrity observed. Mr. Nally informed me that the containment will be broken down and removed from the site either on 4/14/15 or 4/16/15 and any staining discovered on the pad surface will be cleaned up as well.
2409419 Primary Facility 09/08/2015 Routine/Complete Inspection No Violations Noted As I arrived at the site at 12:15, Cabot was in the process of setting up for the production phase of operations and conducting treatment of hydraulic fracturing flowback fluid. Flowback operations have been completed for all wells as of yesterday 9/7/15 according to personnel on-site. Cabot submitted an alternative waste management practice (OG-71) application on 7/28/15 to conduct treatment of flowback fluids generated and transported from the E Strauss wells through an above ground flow line to where the equipment is staged on secondary containment in the tank pad for this treatment process. The secondary containment consists of rig mats, a layer of geotextile felt above and below a High Density Polyethylene Liner made of plastic material and corrugated pipe berm (1) foot in height encompassing the containment. The main function of the treatment process is to remove solid material and adjust the water chemistry of the flowback fluid so it can be effectively reused during hydraulic fracturing of Cabot wells.

Well Pad located in Harford Township, Susquehanna County. All wells permitted for this site have been drilled, stimulated, and appear to be in production. A relatively high number of frack tanks are being stored on the pad. The pad is stable. Good vegetation has been established. Access road ditches are being maintained. No violations noted.
2547590 Primary Facility 2016-12-21 Routine/Complete Inspection No Violations Noted The Department conducted an inspection of the COG Stauss E Well Site on 12/21/16 after having received the Post-Drilling Well Site Restoration Report for this site. Overall, the site is stable with no violations noted. The site was partially snow covered with temporary BMPs still in place. All seven wells associated with this site have been drilled and appear to be in production. The date of the final well drilled was 4/4/2015, the 1H (115-21805).

I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:25p.m.

2742672 Primary Facility 2018-06-14 Routine/Complete Inspection No Violations Noted The Department arrived on site at 1200hrs to conduct a RTNC inspection on the pad. The Department was represented by Gene Rickard (WQS-DEP) and myself.

The required signage was present marking the entrance to the access road. All necessary paperwork was present in the mailbox. The access road was in good condition, along with the associated PCSM BMPs. No apparent erosional issues were noted. The pad's surface was solid and stable. The cut slope and berms on the pad were well-vegetated, solid, and stable.

The wellheads were clearly marked with the well names and permit numbers. There was no visible bubbling or discoloration present in the well cellars.

The Department concluded its inspection and left the site at 1230hrs.

2834830 Primary Facility 2019-01-31 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. As the photo's show, the site is snow covered. Production tank containment should be emptied the next time the stormwater melts, as there is not much free board present. Overall the site has good stabilization. No E&S issues identified. No violations noted.
2846389 Primary Facility 2019-02-28 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was ice 1' below grade in the cellar and I was unable to check for gas. The (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was no gas detected at the poly vent tank with a MSA Altair 5X multi gas detector that was calibrated on 2/5/19. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2855310 Primary Facility 2019-03-13 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Jon Sassi, Department EPS was also present during the inspection. As photo #1 shows, a slip has developed on the cut slope of the well pad. It appears that tire track indentations were the cause of this small slip. Please repair immediately. As photo #4 shows a small oil/grease stain was observed next to the 5H well head. Please remediate immediately. Photo #5 shows that recent plowing has uncovered residual felt liner. Please remove this debris. Other than the three items listed above, overall the site appeared stable with no violations noted.
