| On May 3, 2011, I stopped at the Captain Planet site in Amwell Township to check on the progress toward addressing the violations previously found.  I met primarily with Robert Rikeman of Rice Energy and Jack Cooper of Geological Engineering Services, a Rice contractor.
+
| REUSE (AT WELL PAD)

Rice has finished the vertical drilling bore and is waiting to start the horizontal drilling.

The site is much better and good vegetative growth.  The replacement/repair of the BMPs goes on.

The area on the west of the pad is still bothersome because the topsoil stored there is so wet it causing problems on both sides of the access road.  They asked if they could give some of the excess back to the landowner.  I talked it over with Vince Yantko and as long as they have a adequate supply they can give it away.

The continuing wet weather is still causing problems but the site is mostly seeded and they are hoping to finish the rest soon  I took several photographs and left..

| Checked the clean-up progress.  The site is greening up, the E&S plan is on-site but does need updating.  The well is within 10 feet of the coordinates Rice gave.  Some of the sediment controls need maintenance and the sediment trap is still unresolved.

| The E&S controls are, in all, very good.  Most of the slopes have 90% vegetative cover and the fences and socks are in decent repair.  Rice enlarged the sediment trap and cut a new access road to it to maintain the trap.  The road runs along the south side of the pad and will do double duty as a stormwater channel.  Much of this work has only been done in the last couple of weeks and the newly cut slopes are not yet vegetated.  Additionally, Rice added rock to the sediment traps spillway.  Although the newly expanded trap and access road are still bare, they should green-up shortly.

| REUSE AT A WELL PAD OUTSIDE PENNSYLVANIA

Additionally, the site﾿s E&S plan needs to be updated and initialed.

Because all the construction is completed and most of the site is now vegetated, I am closing out the Captain Planet﾿s violation as of August 3, 2011.  A final Consent and Agreement will also include the Hulk site.

| Checked the clean-up progress.  The site is greening up, the E&S plan is on-site but does need updating.  The well is within 10 feet of the coordinates Rice gave.  Some of the sediment controls need maintenance and the sediment trap is still unresolved.

| Today I conducted a routine inspection at the Captain Planet well pad and the following was observed.
 +
 +
Proper signs were displayed identifying the site. The site was stabilized with thick vegetation no erosion observed outside of the LOD. The well heads were maintained, no leaks or fluids around well head. The production tanks were in good condition no leaks , rust or defects observed. Secondary containment for the site was intact and maintained.

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1970206 Primary Facility 04/14/2011 Compliance Evaluation Outstanding Violations - No Viols Req'd Good progress made. Still work to be done.
1970277 Primary Facility 05/03/2011 Compliance Evaluation Outstanding Violations - No Viols Req'd On May 3, 2011, I stopped at the Captain Planet site in Amwell Township to check on the progress toward addressing the violations previously found. I met primarily with Robert Rikeman of Rice Energy and Jack Cooper of Geological Engineering Services, a Rice contractor.
1976386 Primary Facility 04/20/2011 Drilling/Alteration No Violations Noted Phoenix 7 - TD @ 220, waiting on cement dry time, cement fell back 40' back side grouted with pour string, site and location good, closed loop.
1976400 Primary Facility 04/28/2011 Drilling/Alteration No Violations Noted Phoenix 7 - TD @ 2823, drilling 8 1/2 " vertical hole ahead, returns on cement, site and location good, closed loop.
1983615 Primary Facility 05/12/2011 Drilling/Alteration No Violations Noted HWD 7- TD @ 5483, tripping in hole after bit change, site and location good, closed loop
1992586 Primary Facility 03/21/2011 Complaint Inspection Violation(s) Noted Five violations recorded. Inspection in response to Complaint # 277691.
1992724 Primary Facility 05/18/2011 Routine/Partial Inspection Outstanding Violations - No Viols Req'd Horizontal drilling was continuing. Some of the topsoil was moved but it was still quite wet. Rice had done some road work. Violations described March 21, 2011 still remain.
1992729 Primary Facility 06/09/2011 Routine/Partial Inspection Outstanding Violations - No Viols Req'd The entrance to the pad was rocked and the overall condition of the pad was much better with some of the reseeding taking off. Work still needs to be done on the sediment trap.
1992738 Primary Facility 06/30/2011 Routine/Partial Inspection Outstanding Violations - No Viols Req'd Checked the clean-up progress. The site is greening up, the E&S plan is on-site but does need updating. The well is within 10 feet of the coordinates Rice gave. Some of the sediment controls need maintenance and the sediment trap is still unresolved.
1992746 Primary Facility 07/29/2011 Compliance Evaluation No Violations Noted The E&S controls are, in all, very good.
1995661 Primary Facility 05/24/2011 Routine/Complete Inspection Violation(s) Noted
2008274 Primary Facility 09/30/2011 Routine/Partial Inspection Violation(s) Noted Responded to company-reported oil spill. Barel of used oil knocked over. Good response after the fact.
2016398 Primary Facility 09/12/2011 Compliance Evaluation No Violations Noted viols corrected from inspection 1995661, enforcement 273754 for unit 1H
2080126 Primary Facility 06/26/2012 Routine/Complete Inspection No Violations Noted
2163037 Primary Facility 04/05/2013 Routine/Complete Inspection No Violations Noted Checked paperwork and E&S controls. No violations found.
2260020 Primary Facility 03/24/2014 Complaint Inspection No Violations Noted Rice Energy producing well. Fluid tanks are diked and in good condition. Well heads appear to be in good condition.
2669349 Primary Facility 2017-11-29 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the Captain Planet well pad and the following was observed.
