Pressure gauge on the top of the xmas tree reads 131 PSI, casing pressure gauge reads 331 PSI. 2-3/8" plunger lift is set on this well. 0% combustible gas was detected at the annulus or in the area immediately surrounding the well. No obvious concerns or violations are noted at time of inspection. Wellsite looks neat and well organized.

The 10" poly pipeline has been repaired. The contractor has backfilled, graded, seeded, and mulched all disturbed areas. Compost sock remains in place. A RCE remains in place off the edge of the well pad near the 911 sign. This is how the contractor was accessing the ruptured pipeline with heavy equipment. In addition, the contractor as added some rock to a nearby ditch line (see photos). All roll off boxes have been removed from the site. An excavator remains onsite.

Please note this well pad and pipeline were previously constructed with the previous operator, EOG Resources. The pipeline construction was covered under conservation district permit ESCGP1-1709801. This ESCGP covered multiple well pads and pipeline phases. Department personnel conducted notice of termination (NOT) inspections for the ESCGP during 2013 and approved the NOT.

Contractor S&T Excavating was onsite removing soil from the pipeline rupture which occurred on the morning of March 15, 2019. The contractor has been placing the excavated soils on the D16-PHC- L well pad. I viewed the exposed 10" poly pipeline break. Annular vent (7-5/8" x 4-1/2") is closed and not vented, plumed to the tank. Pressure gauge on the top of the xmas tree reads 131 PSI, casing pressure gauge reads 331 PSI. 2-3/8" plunger lift is set on this well. 0% combustible gas was detected at the annulus or in the area immediately surrounding the well. No obvious concerns or violations are noted at time of inspection. Wellsite looks neat and well organized. Seneca Resources Corp. 2819749 Primary Facility 2019-04-02 Follow-up Inspection No Violations Noted Department representative Sue Herzing conducted a follow-up site inspection at Seneca Reources D16-PHC-L well pad and nearby pipeline on April 2, 2019.

The 10" poly pipeline has been repaired. The contractor has backfilled, graded, seeded, and mulched all disturbed areas. Compost sock remains in place. A RCE remains in place off the edge of the well pad near the 911 sign. This is how the contractor was accessing the ruptured pipeline with heavy equipment. In addition, the contractor as added some rock to a nearby ditch line (see photos). All roll off boxes have been removed from the site. An excavator remains onsite.

2851067 Primary Facility 2019-02-21 Routine/Complete Inspection No Violations Noted The weather is 34 Deg. F and sunny at time of inspection. Routine inspection of Seneca Resources Corp., D16-PHC-L Pad, Well # 3H on February 21, 2019. There is 1 active well on this pad.

This well is open and producing at the time of inspection. Annular vent (7-5/8" x 4-1/2") is open and plumed to the tank. 0% combustible gas was detected in the area immediately surrounding the well. Production tubing pressure is 138 PSI, backside casing pressure 203 PSI. Production processing equipment on location includes 1 production water tank, 1 gas processing unit, measurement equipment ¿ orifice meter stations. No obvious concerns or violations are noted at time of inspection. Wellsite looks neat and well organized. Seneca Resources Corp. Representative - Stephen Gus Simpson, Roustabout Foreman. 2855301 Primary Facility 2019-03-18 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at D16-PHC-L well pad on March 18, 2019 and nearby pipeline with Seneca representative Ryan Lucente and Larry Swanson. Please note this well pad and pipeline were previously constructed with the previous operator, EOG Resources. The pipeline construction was covered under conservation district permit ESCGP1-1709801. This ESCGP covered multiple well pads and pipeline phases. Department personnel conducted notice of termination (NOT) inspections for the ESCGP during 2013 and approved the NOT.

Contractor S&T Excavating was onsite removing soil from the pipeline rupture which occurred on the morning of March 15, 2019. The contractor has been placing the excavated soils on the D16-PHC- L well pad. I viewed the exposed 10" poly pipeline break. According to Mr. Lucente and Mr. Swanson, the 10” poly pipe broke where the drip line support is located at. When viewing photos 1&2 the top black pipe is the 10" poly gas line. The bottom black pip
