| On this day, I am conducting an inspection to document the SIMOPS progress for the plug and abandonment of permit number 4446. I spoke with Antero representative Barney Etcher, and he stated the SIMOPS process is being delayed due creating a standard operating procedure between departments within Antero Resources. Containment is being set in place and steel cages for the producing wells are onsite. The well in question currently has a nightcap in place and no gauges are present. I found the pad gated with signage at the entrance. There are (6) producing wells, (6) gas processing units (GPU), (4) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi.

Concrete barriers surround the wellheads. The facility is gated with an electronic shutdown button on the right-hand side, prior to the pad. The front left of the pad is equipped with (8) 400 barrel steel tanks within containment and an electronic shutdown button. There are (6) gas processing units along the front right side of the pad, and a midstream facility along the front right side, as well.

The well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure gauge reads (0) zero pressure and the 9.625 casing well head reads 600 pounds pressure. On pad total is (6) complete wells supported by (6) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area.

They ran a 18.625''guide shoe and a float collar. They ran centrilizers on the 1st, 2nd, 4th and 6th joint. The casing was ran to depth of 378'. Allied ran 189 barrels of water ahead and broke circulation. They than started gel slurry of 25 barrels. After running a 10 barrel water spacer then they started mixing cement. Allied mixed 159 barrels of cement slurry of Class A cement with 2%cacl and .25 lb/sack of flow seal. Plug was dropped and displaced down then plug was bumped and taken pressure up to 650 psi for 5 minutes and held an released back and shut in. There were 70 barrels of returns back to surface. Water was at 60 degrees Fahrenheit and Ph was at 7. Good surface job.

0095465901 2019-01-24 Production Wells No Violations On this day, I am conducting an inspection to document the SIMOPS progress for the plug and abandonment of permit number 4446. I spoke with Antero representative Barney Etcher, and he stated the SIMOPS process is being delayed due creating a standard operating procedure between departments within Antero Resources. Containment is being set in place and steel cages for the producing wells are onsite. The well in question currently has a nightcap in place and no gauges are present. I found the pad gated with signage at the entrance. There are (6) producing wells, (6) gas processing units (GPU), (4) 400-barrel steel production tanks within polymer lined steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Additionally, concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 0 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 0 psi.
0254739269 2018-03-07 Production Wells No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please refer to his comments for a detailed report. The well is equipped with annular pressure monitoring devices. Concrete barriers surround the wellheads. The facility is gated with an electronic shutdown button on the right-hand side, prior to the pad. The front left of the pad is equipped with (8) 400 barrel steel tanks within containment and an electronic shutdown button. There are (6) gas processing units along the front right side of the pad, and a midstream facility along the front right side, as well.
0477257962 2018-03-07 Production Wells No Violations Status check inspection accompanied by ODNR Inspector Tom Thomas to document the well status. The well is equipped with a casing and tubing well head assembly with (2) manually operated valves on the tubing well head and one emergency shut down valve on the tubing tree production flow line. The well head casing flow line is also tied into the production flow line however the valves are in the closed position. The 13.375 and 9.625 casing well head assemblies have flowlines with valves and gauges installed to monitor annular pressure. The 13.375 casing well head pressure gauge reads (0) zero pressure and the 9.625 casing well head reads 600 pounds pressure. On pad total is (6) complete wells supported by (6) gas processing units. Also on location is a production fluid facility consisting of a total of (8) steel construction tanks with a capacity of 400 barrels of fluid each, the tanks are situated inside a polymer lined steel containment dike area.
0526972550 2014-04-17 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress found the current drilling depth to be 6,535 in the 12.250 2nd intermediate well bore section with the drilling company pulling out of the hole for a bit change.
0746710867 2015-06-17 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well location has proper erosion control and vegetation established. The company has recently added additional earthen containment around the well pad perimeter. Addressing the well status the well drilling and completion process is complete with the well in production with additional gas processing equipment located on the well pad in a separate location from the producing wells, there is also a total of 8-400 barrel capacity production fluid tanks on location located inside a plastic lined metal dike area with proper well identification posted at the well head. Preliminary Restoration passed on this day.
0838142651 2019-01-29 Production Wells No Violations On this day, I found Antero progressing with the SimOps procedure in preparation for the plug and abandonment of permit number 4446. TekSolv is onsite installing back-pressure valves in the tubing hanger on the (6) producing wells on the pad, prior to installing the steel cages over the wellheads. Fortis Energy is also onsite installing containment and preparing to rig-up, once the SimOps procedures have been completed. I spoke with Antero Representative Barney Etcher during the inspection.
1047601858 2015-02-09 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well is now in production with a gas processing unit in place on location associated to the said well. There a quantity of 8- 400 barrel production tanks on location supporting the 6 producing wells now located on the well pad. Preliminary Restoration was passed on this day.
1375565082 2014-06-19 Drill / Deepen / Reopen No Violations Status check inspection on the drilling progress found Patterson Drilling rig 342 on location drilling the 8.5 production borehole at a depth of 13,048 using a14.2# drilling mud with a directional azimuth of 147.92 degrees and inclination of 88.89 degrees, borehole circulation was lost this morning and the company has pumped approximately 1200 barrels of drilling mud into the borehole while lightening the weight to 13.8 # with the pump pressure increasing indicating fluid fill up, 3rd party gas influx is minimum and the drill string is free.
1388432524 2013-12-30 Casing String Witnessed No Violations The said well was spudded on 12/29/13 by Express Energys top hole rig. On 12/30/13 Express Energys cement services cemented 100 of 26 conductor to surface with 10 barrels of cement returns to surface.
1390345463 2014-01-21 Drill / Deepen / Reopen No Violations Status check inspection on the current drilling progress found Falcon rig 20 drilling the 17.5 1st intermediate casing at a depth of 1872 with a proposed total depth of 2100. The drilling rig is currently not operation due to rig motor being broke down.
1390545918 2014-01-24 Casing String Witnessed No Violations Status check inspection on the drilling process found Falcon Drilling rig # 20 has reached the total depth to be drilled on the 17.5 1st intermediate well bore at a depth of 2159 and is currently running a total length of 2070 of 13.375 54.5# J-55 casing in the hole with a casing shoe and float collar used along with proper centralizer placement demonstrated and Allied cement services standing by to cement the casing to surface with a proposed 1846 sacks of Class A- 2% Calcium 1/4# flake per sack cement with 50% open hole excess proposed.
1391187137 2014-01-18 Casing String Witnessed No Violations On the Franklin 2H job there was 100 of 26conductor casing at 154.4lb/ft already cemented in place. The open hole of 24 was drilled to 398. The 18.625is 87.50 lb/ft. of j-55 casing. They ran a 18.625guide shoe and a float collar. They ran centrilizers on the 1st, 2nd, 4th and 6th joint. The casing was ran to depth of 378. Allied ran 189 barrels of water ahead and broke circulation. They than started gel slurry of 25 barrels. After running a 10 barrel water spacer then they started mixing cement. Allied mixed 159 barrels of cement slurry of Class A cement with 2%cacl and .25 lb/sack of flow seal. Plug was dropped and displaced down then plug was bumped and taken pressure up to 650 psi for 5 minutes and held an released back and shut in. There were 70 barrels of returns back to surface. Water was at 60 degrees Fahrenheit and Ph was at 7. Good surface job.

1403745924 2014-06-25 Casing String Witnessed No Violations Status check inspection on the 5.5 long string cement job, 15,749 of 23# casing was ran with a guide shoe and float collar on the bottom along with proper centralizer placement demonstrated, 1609 sacks of class H 15.2 ppg-1.18 yield cement was used with 20% calculated open hole excess and a calculated top of cement on the annular side of the 5.5 production casing at 1000 above the top of the horizontal curve kick off point.
1480039080 2014-08-25 Completion Testing No Violations Status check inspection of the well pad post stimulation found the company men on site and the old containment has been removed and the location is being leveled in preparation of installing new containment prior to commencing stimulation, a suggestion was made to improve the well pad site containment in the area behind the fluid separators.
