| I was on location to witness Cemenco set a Berea plug (1771-1371) and a bottom of surface plug (240-0). There was circulation on all plugs and some oil. They used a total of 35 sacks of Class A cement. We used Appalachian Well Services to log the hole and we were going to shoot the 4 1/2 inch diameter pipe off but no free point could be found. When we logged the hole it was found that the 4 1/2 inch diameter casing was cemented on the back side almost clear to surface. There was no 7 inch diameter surface casing in the hole but we found there was a 12-13 inch hole at the surface that was about 15 feet deep. After we plugged to surface we took a piece of tubing and put it in the previously mentioned hole and filled that to surface with 15 sacks of cement. This well is now plugged and all violations have been resolved.
