| I was on location to check the status of the mine string being drilled on this location with an auger rig. A 30 conductor was set to 60' and they expected the mine void to be hit around 115' below the surface. The auger rig will drill the mine string and set 20 welded casing into the mine void. I had to leave the location for a surface cement job before I could witness the job.

| I was on location to check the status of the mine string being drilled on this location with an auger rig. A 30 conductor was set to 60 and they expected the mine void to be hit around 115 below the surface. The auger rig will drill the mine string and set 20 welded casing into the mine void. I had to leave the location for a surface cement job before I could witness the job.

| The purpose of my inspection was to conduct a status check on well #2-1799.  The lease road and well pad remained in good repair.  The topsoil stockpile remained stable.  A locked chain link fence was constructed around the facility.  No signs of vandalism were present.  A stable earthen berm was present outside of the fenced perimeter.  Correct identification was present.  The well cellar was backfilled with gravel.  No leaks were detected.  The 9.625 annular pressure gauge indicated 0 psi.  The 5.5 annular pressure gauge indicated 200 psi.  The well was flowing during the inspection.  The 5.5 production casing was equipped with a plunger lift assembly.  Production equipment on the pad consisted of (1) sand knockout and GPU,  one (1) hydrate inhibitor tank, one (1) meter run and three (3) 400 bbl produced water and three (3) 400 bbl condensate tanks inside steel containment.  The containment was lined with an impervious liner and was holding fresh water.  No violations were observed during this inspection.

--Tex Pumping LLC was the cementing contractor. There was good fluid circulation throughout the cementing operation. The cement was mudscaled at 15.8 pounds per gallon (ppg). O-Tex adjusted the cement density, and pumped 79 bbls of cement slurry at 15.6 ppg.  An estimated 10 bbls of cement returned to surface, and were mudscaled at 15.5 ppg.  The cement was displaced with 59 bbls of water.  Displacement was completed at 12:57am on 06/08/2013.  Approximately 10 of cement were left inside the casing. The valve on the top of the casing was shut in.

-based mud used to drill and condition the hole, a lead cement followed by a tail cement were pumped.  The lead and tail slurries were mudscaled, and Schlumberger pumped 273 bbls of lead cement at 13.5 pounds per gallon (ppg), and 333 bbls of tail cement at 15.0 ppg.  The plug was dropped and displaced with 307 bbls of water as measured by pump truck tanks.  The plug bumped the float collar at 9:32pm on 06/29/2013.  The plug landing pressure was 2204 psi, and it was raised to 2786 psi and held for 5 minutes.  Pressure held.  The float held when the pressure was released, and 3 bbls of water bled back.

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0111819665 2012-11-16 Surface Facility Construction No Violations I went to the site and conducted a pre-construction site inspection with Steve Ault, Chesapeake Field Representative.
0323961220 2013-06-05 Casing String Witnessed No Violations
0649678905 2013-06-06 Drill / Deepen / Reopen No Violations I was on location to check the status of the mine string being drilled on this location with an auger rig. A 30 conductor was set to 60 and they expected the mine void to be hit around 115 below the surface. The auger rig will drill the mine string and set 20 welded casing into the mine void. I had to leave the location for a surface cement job before I could witness the job.
0794507285 2017-08-09 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1799.

1707697631 2016-02-19 Production Wells No Violations I was on location for a status check. My inspection found the access road gate locked and the

access road is in good repair. An identification sign at the access road entrance, well, and tank battery. The pad is fenced and all man doors are locked. The #1H well is in production. The well is equipped with a christmas tree valve assembly. There is a steel dike around the tank battery. The equipment on the pad consists of the following: 6 - Storage tanks 400 bbl 1 - Vapor recovery unit (VRU) 1 - Gas processing units (GPU) 1 - Vertical separator 1 - Sand separators 1 - Thermal electric generator 1 - Compressor 1 - Producing wells Site fenced and man doors locked

1874562523 2016-03-03 Preliminary Restoration No Violations I was on location for a status check. My inspection found the access road gate locked and the access road is in good repair.
