| Drill / Deepen Reopen
| I inspected this producing unit as part of routine inspection activity. The well was completed in 03/10/2005. The unit is producing and production reports are current.  Elements of the well consist of a production wellhead with standard valve assembly and pump jack system and aboveground storage tank (AST) battery. The wellhead and surrounding area are free of signs of leaks or  other impacts. The wellhead is located across from the Lake Metro Parks Pleasant Valley Park between Pleasant Valley Road and the Chagrin River. The area surround the wellhead and battery is wooded. Access to the wellhead is via a dedicated gravel access road which travels north from Pleasant Valley Road, and passes the well in 300 ' before looping back to the main road.  The battery is located south of Pleasant Valley Road via a similar gravel access road which also accesses the Metro Park parking. The battery is comprised of 1 steel 100-barrel production tank, 1 100-barrel steel brine tank, and separator unit. Spacing from well to battery, and spacing of elements within the battery meet regulations. The load-outs are bull-plugged. The interior portion of the battery is overgrown with vegetation.  I did not observe any signs or evidence of leakage, spills, or problems. Surface drainage from the area is towards the Chagrin River, 400 feet north. I did not observe any gully formation or other surface drainage erosional deficits. The ID placards at the wellhead and battery are in good condition.  I notified Alliance about the vegetation overgrowth in the battery.

The wellhead is located across from the Lake Metro Parks Pleasant Valley Park between Pleasant Valley Road and the Chagrin River.  The area surround the wellhead and battery is wooded. Access to the wellhead is via a dedicated gravel access road which travels north from Pleasant Valley Road, and passes the well in 300  before looping back to the main road.

| I inspected this producing unit as part of routine inspection activity. The well was completed in 07/08/2005. The unit is producing and production reports are current.  Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger-lift system and aboveground storage tank (AST) battery. The wellhead and surrounding area are free of signs of leaks or other impacts. The wellhead and battery are located in a sparsely developed portion of Willoughby Twp.  The area surround the wellhead is primarily a low-land meadow adjacent to the Chagrin River.  Access to the wellhead and battery are via a dedicated gravel access road which travels south from Pleasant Valley Road Metro Park  The battery is comprised of 1 steel 210-barrel production tank, 1 100-barrel poly brine tank, and separator unit. The load-out is bull-plugged. The secondary containment at the battery appears well maintained. Surface drainage from the area is to the north, towards the Chagrin River, 300 mile east. I did not observe any gully formation or other surface drainage erosional deficits. The ID placards at the wellhead and battery are in good condition. I notified Alliance about the need for signage at the gated entry to the area and for spacing compliance at the battery.

| My inspection today found a plunger lift system well.  The well was in good repair.  The tank battery had a 100 barrel production tank, a 1500 gallon brine tank and a mechincal separator in it.  The locks on the man door to the tank battery was rusted shut.  The south east corner of the tank battery needs built up.  Operator will be contacted and informed of the needed repairs.

| I inspected this well as part of routine inspection activity. The well and battery are located on a parcel that is largely undeveloped.  At the time of this inspection, I found the following: a wellhead plunger-lift unit and associated battery in separate chain-link enclosures. The fittings and connections were secure and free from leakage or problems. Fittings and connections at the battery were secure. The load-lines were plugged and locked. ID signage was present and current. The secondary containment was intact with no signs of leakage, spills, or problems. The shared-use access road was in good condition. The well was in active production. The chart recorder was active. Production reports and insurance were current.
