| On the day of my inspection I collected a soil sample from inside the dike. I mixed the sample with dH20 and tested it for chlorides with a Quantab test strip. The chloride level was <32 ppm. I found that the agricultural ditch had been restored to its original course. At 11:30 I collected a sample of the water from the ditch and tested it with a Quantab. The chloride level was at 50 ppm.

| I noticed a strong petroleum odor when I drove by this tank battery at 09:30. When I stopped to investigate I found a pool of water in the corner of the dike, just below the valve on the 100 barrel tank. The valve was closed and there was a bull plug in place. I noticed that there was evidence of the water running over the dike, down the lease road and into the roadside ditch. I took samples from each of the fore mentioned locations and tested them for chlorides with Quantab test strips. All three tests were off the scale with the low range Quantab. The fluid tested between 2100 and 2800 parts per million (ppm) with the high range Quantab.

At 09:45 I called James Miller, who met me at the site by 10:00 and admitted to opening the valve on the 100 barrel tank at approximately 09:00. He claimed that he released approximately 10 barrels or 420 gallons. Miller claimed that He thought it was fresh water. I reported the release to the Ohio EPA Spill Hotline at 10:25. Ohio EPA field inspector Trevor Irwin, Ohio wildlife officer Brad St.Clair, and Trisha Cartmell from DOGRM Emergency Operations also responded to the incident.

The roadside ditch led to a culvert that directed water across CR 10 into an agricultural drainage ditch. I tested the water at two locations along the agricultural ditch and still found the water to be above 2000 ppm. I then took a sample where the agricultural drainage reconnected with roadside drainage and dumped into an intermittent stream. The water tested at 1131 ppm chlorides. I went to the point where the intermittent stream drains into White Eyes Creek. I found no chlorides. I tested the water in the intermittent stream 100 meters downstream from where the roadside drainage dumped in. I found less than 32 ppm.

| On the day of my inspection I met James Miller at the agricultural drainage ditch at 08:15. I found that there was still a steady stream of water flowing down the ditch. Miller claimed that they took one load of water out of the sump on the evening of 6/9/15 and took another out this morning. I tested the water flowing down the ditch with a Quantab test strip and found the chloride level to be at 78 parts per million (ppm). I then went to the tank battery where the fluid was initially released. There was no standing water available to sample. I collected a sample from the head of the agricultural ditch, approximately 200 feet from CR 10. The Quantab test strip showed 490 ppm chlorides. I instructed Miller to bring fresh water in a clean tank to flush the ditch again. They brought 10 barrels of fresh water. I tested the water prior to release, and it did not even register on the Quantab test strip. At 13:00 they released the fresh water in the roadside ditch at the tank battery. We then went to the end of the drainage ditch where the dam and sumps were constructed. I waited 30 minutes and tested a sample of the water. The test strip showed 71 ppm chlorides. I waited another 30 minutes, and tested again. The chloride level rose to 148 ppm. I waited another 30 minutes and the chloride level was down to 78 ppm. I told Miller to fill in the sump, open the dam and restore flow through the agricultural drainage ditch.

| I stopped to investigate a bare swath of soil outside of the containment dike because I had an incident in 2015 where I found Miller Oil draining water out of their tank. On the day of my inspection I found that the line to the poly brine tank was secure and there were no signs in the dike that any fluid had been drained on site. The bare soil turned out to be a turn around spot to get vehicles back onto CR 10 safely.
