| I was on location to follow up to NOV #1140145497. Inspector Dave Claus issued a NOV on 02/16/2006 for the well to be plugged. There has been zero production reported for this well. This well is a potential safety hazard. It is located very close to a college softball field and it is not fenced off. The owner previously agreed to put this well into production and put a new ID sign on the well. The wellhead is currently rusted and the flow lines from the well are open without any valves or bull plugs. This well is currently incapable of production. This well must be plugged.

--139 requiring the plugging of this well.  The well has not been plugged.  The main and side gate valves are closed.  The well is disconnected from the production line.  The production line away from the well is open.  No known tank or tank battery has been located for this well.  The well is not legibly identified and there are no annual production reports submitted. The well has peeling paint and rust on it.  I called 330-280-3167 and left a message for Mr. Svetlak on 04-12-2017. Complaint # 5242 is associated with this well.  I contacted and spoke with the complainant on 04-10-2017.

| I was on location to follow up to compliance notices issued for this well and agreements for work to be done stemming from a meeting on May 16, 2014 with the operator. This well is a potential safety hazard. It is located very close to a college softball field and it is not fenced off. It was agreed upon that within 3 weeks identification be posted at the wellhead and a fence/cage be put around the wellhead for general safety. At this time none of this work has been completed. Stephen Svetlak was given 6 months to buy a sectional line from Dominion East Ohio Gas and put this well into production. This well must have identification and a fence/cage put on it in the meantime for safety.

This well is a potential safety hazard. It is located very close to a college softball field and it is not fenced off. The well needs a correct I.D. sign and the well is idle (zero reported production). Plug the well or produce and fence off the well. Jay Cheslock notified Steve Svetlak today of the Colleges concerns. Svetlak stated that he would address the N.O.V. (idle well, I.D. sign, and fencing) and put the well in production.
