| At 8:54 AM, I called Mr. Bud Porterfield to discuss landowner complaint and status of well(s). Mr. Porterfield confirmed non-production for several years and has focused his attention on wells in Kentucky.  He mentioned non-related wells and not on Stevens property, in which he let a guy have these wells and called him a year ago and nothing was done.  Informed that wells still remain under his name and when asked if there would be an assignment he stated at Licking Co. Recorders office he stated that attorney took care of transfer and was not sure.  Mr. Porterfield stated that he talked to 3-4 people about wells on Stevens property and will talk with his attorney today. Informed Mr. Porterfield of SB 165, a recent change to Ohio oil and gas law and asked for cooperation. Mr. Porterfield agreed to work with the Division to achieve compliance and will talk with attorney today.  At 3:28 PM, I had a missed call from Mr. Porterfield.  I returned his call and answered questions about well ownership transfer and provided Erica Freeman's contact number at Central Office. Mr. Porterfield would like to sell all wells on Stevens property to company and will also work on transfer of wells on non-related property. The wells listed under Wallick Petroleum and Unknown Owner, on Stevens property, were also brought to his attention, in which he confirmed ownership.  Central Office will be notified of ownership status.

| The Division received a landowner complaint alleging all Berea wells owned by Bud Porterfield have been idle for approximately 12 years.  An inspection revealed an idle well with a pumping unit, motor and no electric.  No house gas received from this well.  The polish rod showed no signs of production and flowline from pumping tee was not connected to the tank battery.  Note: Tank battery consisting of (3) 100 bbl tanks are located opposite side of road on SR 586.  A GPS reading obtained for the well.  No identification observed at well or tank battery.  At time of inspection, this well was listed under previous well owner, Wallick Petroleum Co., and not transferred to Bud Porterfield.  Mr. Porterfield confirmed well ownership and the landowner has also stated that he owns the well.  Mr. Porterfield will need to complete Form 7A for well ownership per Central Office.  Records indicate no production statements submitted since 1998, which had shown zero for oil and gas.  On 4/9/2012, at 8:54 AM, Mr. Porterfield was verbally informed of complaint and status of well(s) and instructed to plug or produce.  Furthermore, additional wells would need to be inspected to cover all wells on Stevens property.  Mr. Porterfield stated that he sold wells 2-3 years ago on non-related property to a guy and called 1 year ago and nothing done; No assignment at Licking Co. Recorders Office; Spoke with 3-4 people about wells on Stevens property; and would talk with his attorney today.

| This inspection was conducted as a production well status check. The well was not equipped with a pumping unit. It was not capable of production. Identification was not posted at the well. H & A Drilling has failed to report production on this well since taking ownership. The landowner, Mr. Brad Stevens, filed a complaint with the Division regarding nonproduction of the wells on this lease in 2012. The well owner has changed since the complaint was filed and the new owner has failed to bring well into compliance.The well owner/operator shall submit a written plugging schedule to Brandon Mitchell at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per ORC 1509.12(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. Time of inspection: 1:50 PM. Weather: 40*F, Sunny

| My inspection revealed that the well has not been plugged or placed into production. It was not capable of production as there was no pumping unit on the well. I issued a compliance notice on 4/14/2016 to have the well plugged. I took one photo of the well. I left a voicemail message for Carl Bevard at 11:55 AM. I received a phone call from Cheri Bevard at 12:04 PM. Cheri asked what Carl Bevard needs to do to be in compliance. | At 8:54 AM, I called Mr. Bud Porterfield to discuss landowner complaint and status of well(s).  Mr. Porterfield confirmed non-production for several years and has focused his attention on wells in Kentucky.  He mentioned non-related wells and not on Stevens property, in which he let a guy have these wells and called him a year ago and nothing was done.  Informed that wells still remain under his name and when asked if there would be an assignment he stated at Licking Co. Recorders office he stated that attorney took care of transfer and was not sure.  Mr. Porterfield stated that he talked to 3-4 people about wells on Stevens property and will talk with his attorney today.  Informed Mr. Porterfield of SB 165, a recent change to Ohio oil and gas law and asked for cooperation.  Mr. Porterfield agreed to work with the Division to achieve compliance and will talk with attorney today.  At 3:28 PM, I had a missed call from Mr. Porterfield.  I returned his call and answered questions about well ownership transfer and provided Erica Freemans contact number at Central Office. Mr. Porterfield would like to sell all wells on Stevens property to company and will also work on transfer of wells on non-related property.  The wells listed under Wallick Petroleum and Unknown Owner, on Stevens property, were also brought to his attention, in which he confirmed ownership.  Central Office will be notified of ownership status.

1447079114 2012-04-09 Administrative Inspection No Violations At 8:54 AM, I called Mr. Bud Porterfield to discuss landowner complaint and status of well(s). Mr. Porterfield confirmed non-production for several years and has focused his attention on wells in Kentucky. He mentioned non-related wells and not on Stevens property, in which he let a guy have these wells and called him a year ago and nothing was done. Informed that wells still remain under his name and when asked if there would be an assignment he stated at Licking Co. Recorders office he stated that attorney took care of transfer and was not sure. Mr. Porterfield stated that he talked to 3-4 people about wells on Stevens property and will talk with his attorney today. Informed Mr. Porterfield of SB 165, a recent change to Ohio oil and gas law and asked for cooperation. Mr. Porterfield agreed to work with the Division to achieve compliance and will talk with attorney today. At 3:28 PM, I had a missed call from Mr. Porterfield. I returned his call and answered questions about well ownership transfer and provided Erica Freemans contact number at Central Office. Mr. Porterfield would like to sell all wells on Stevens property to company and will also work on transfer of wells on non-related property. The wells listed under Wallick Petroleum and Unknown Owner, on Stevens property, were also brought to his attention, in which he confirmed ownership. Central Office will be notified of ownership status.
1782307190 2017-03-21 Production Wells No Violations Inspect well as a compliance notice follow-up. The inspection revealed that the well has not been plugged or placed in production. The well was not capable of production at the time of inspection. 1889674012 2018-10-24 Production Wells No Violations My inspection revealed that no attempt has been made to bring well into compliance. It was not capable of production as there was no pumping unit on the well. I issued a compliance notice on 4/14/2016 to have the well plugged. A request for enforcement action was submitted to DOGRM management in September of 2018. I two photos of the well during this inspection. I placed a phone call to Mr. Carl Bevard at 1:40 PM but did not get an answer. Mr. Bevards voicemail box was full and I could not leave a message. Time of inspection: 1:00 PM.
