| I inspected this well as a follow-up to a previous inspection and compliance notice in which I had noted  The well is located in a largely heavy industrial area. At the time of this inspection, fittings and connnections at the well and battery were secure. The load-line valves were plugged and locked. The secondary containment was intact. The tank which I had observed with holes had been removed. I observed no other signs or problems or issues.

| I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: A well with pump jack system and associated tank battery. Fittings and connections at the wellhead were secure and free from signs of leakage or problems. The tank battery was enclosed in a chain-link enclosure which was locked and secure. Secondary containment was intact and free from signs of spills or leaks. I noted the following deficiency: the 210-barrel steel production tank was severely corroded with numerous perforating holes which indicates failure of the tank integrity. The well owner must remove the tank from service and replace with a competent tank. These activities must be completed by 12/16/2016. At the time of this inspection, fittings and connnections at the well and battery were secure. The load-line valves were plugged and locked. The secondary containment was intact. The tank which I had observed with holes had been removed. I observed no other signs or problems or issues.

The battery is comprised of 1 210-barrel steel AST, 1 100-barrel AST, mechanical separator. ID is missing. Secondary containment is adequate. The wellhead is fitted with a mechanical plunger lift system and is producing. The wellhead is not enclosed. I did not observe any leaks or problems.

The well is located in a heavy industrial portion of Cleveland between I-77 and the Cuyahoga River. The wellhead plunger lift system is functioning and free of signs of leakage or other problems. 1479310995 2016-11-16 UrbanProduction Wells Violations Noted I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: A well with pump jack system and associated tank battery. Fittings and connections at the wellhead were secure and free from signs of leakage or problems. The tank battery was enclosed in a chain-link enclosure which was locked and secure. Secondary containment was intact and free from signs of spills or leaks. I noted the following deficiency: the 210-barrel steel production tank was severely corroded with numerous perforating holes which indicates failure of the tank integrity. The well owner must remove the tank from service and replace with a competent tank. These activities must be completed by 12/16/2016.
