The wellhead and battery are located in parking lot of the high-rise apartments at the referenced address.  Access to the well is via the paved driveway which accesses the apartment facility . The wellhead plunger lift and battery are in separate board-on-board enclosures which are locked. The wellhead is comprised of 1 100-barrel steel lowboy production tank and 1 50-barrel steel lowboy brine tank, separator and Dominion metering run. Fittings and connections are secure and free from signs of leakage or problems. The load-outs are secure, plugged, and locked. Process safety gear is present. Signage is in place and current. At the time of this inspection there are 15 to 20 inches of snow cover in the area which obscures some surficial elements from observation.

At the time of this inspection, I observed the following:  The load lines were plugged and secure. The secondary containment was intact and dry with no signs of leaks or spills. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. I walked the areas surrounding the wellhead and battery, and found no signs of impact or degradation attributable to the production system.

| I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following: The well was comprised of a mechanical plunger-lift system in a board-on-board enclosure. The enclosure gate was locked and secure. Fittings and connections at the wellhead were secure and free from signs of lekage. The tank battery was comprised of one 100-barrel low boy steel production tank, one 50-barrel steel brine tank, horizontal mechanical separator, and associated valve and tubular appurtenances. There were no signs or indications of problems or leakage. Load-line valves were locked and secure. Secondary containment was comprised of a poured-in-place cement berm which was intact. The areas surrounding the well conisst generally of asphalt-paved drive and parking areas, and I did not observe any signs or evidence of problems or environmental degradation.

The wellhead and battery are located in parking lot of the high-rise apartments at the referenced address. Access to the well is via the paved driveway which accesses the apartment facility . The wellhead plunger lift and battery are in separate board-on-board enclosures which are locked. The wellhead is comprised of 1 100-barrel steel lowboy production tank and 1 50-barrel steel lowboy brine tank, separator and Dominion metering run. Fittings and connections are secure and free from signs of leakage or problems. The load-outs are secure, plugged, and locked. Process safety gear is present. Signage is in place and current. At the time of this inspection there are 15 to 20 inches of snow cover in the area which obscures some surficial elements from observation.

At the time of this inspection, I observed the following: The wellhead plunger lift and associated tank battery located in the parking area for the adjacent high-rise apartments. At the time of this inspection, I observed the following: The load lines were plugged and secure. The secondary containment was intact and dry with no signs of leaks or spills. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. I walked the areas surrounding the wellhead and battery, and found no signs of impact or degradation attributable to the production system.

The secondary containment consists of a concrete dike. Unit identification is present and legible at the tank battery. I did not observe any leaks, spills, or other signs of contamination.

The tank battery was comprised of one 100-barrel low boy steel production tank, one 50-barrel steel brine tank, horizontal mechanical separator, and associated valve and tubular appurtenances. There were no signs or indications of problems or leakage. Load-line valves were locked and secure. Secondary containment was comprised of a poured-in-place cement berm which was intact. The areas surrounding the well conisst generally of asphalt-paved drive and parking areas, and I did not observe any signs or evidence of problems or environmental degradation.
