| I was accompanied today by Steve Tomkins, Lake County Inspector, DOGRM. Well stimulation progressed at 8:35 AM and ended at 10:45 AM. Well stimulation was accomplished using 220 Sacks of sand, 1,000 gallons of 15% HCL Acid and a total of 45,041 gallons of freshwater. 20 shots were placed earlier from 5,568 to 5,558. Hydro frac stimulation was completed with no issues or problems. A separate well visitation report is filed in RBDMS.

| I inspected this well as part of routine inspection. Fittings and connections at the well and battery were secure and free from leakage or problems.  The tank lids were closed. The secondary containment was intact and dry.  The individual enclosure gates were locked.  Production reports were current and up-to-date, but the well was not producing at the time of my inspection. The dedicated portion of the shared-use access road was in good condition with some wash-out forming that may require some mitigation. I walked and inspected the areas surrounding the wellhead and battery and found the natural greenscape to be free from signs of impact or degradation. I notified Gonzoil about the possible need for maintenance at the acess road.

| I inspected this producing well as part of routine inspection activity. The wellhead is comprised of a pump jack unit in a chain-link enclosure. The enclosure gate is locked and secure. Fittings at the wellhead are secure and free from leaks or other issues. There is gas being vented from the 8.625-inch surface casing which should be captured by the system. The load-lines require locks on the valves. I notified the owner about the aforementioned issues and they indicated these items will be resolved immediately.

| I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following: a pump-jack system in a locked and secure chain-link enclosure. Fittings and connections at the wellhead were secure and free from leakage. During a previous inspection, I had noted that gas vented from the surface casing nipple should be plumbed back into the system. That condition had been corrected. Connections and fittings at the battery were secure. The load-lines were locked, which had been noted as a deficiency previously. Tank lids were closed and sealed. The secondary containment was intact and free from signs of leakage or other deficiencies. The dedicated portion of the access road which travels north from the terminus of the driveway was noted to be in good condition. No noted deficiencies at the time of this inspection.

| I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with pump jack system and associated tank battery in separate chain-link enclosures located at the back portion of a residential parcel. There is also a indirect heating unit in a board-on-board enclosure located 50 feet north of the wellhead. I found the enclosure gates to be locked and secure, with the following exception: the enclosure gate for the heating unit was broken and not secure. I notified Doug Gonzales about the maintenance needs at the heating enclosure gate. I found the load-line valves to be locked and secure. The tank lids were closed and sealed. The secondary containment was intact and dry. Fittings and connections at the wellhead and battery were secure and free from signs of leakage or problems. I walked the areas around the wellhead and battery which consisted of mixed areas of grass and wooded areas. I did not note any signs of issues or environmental degradation resulting from the on-going production activity.

Well stimulation was accomplished using 220 Sacks of sand, 1,000 gallons of 15% HCL Acid and a total of 45,041 gallons of freshwater. 20 shots were placed earlier from 5,568 to 5,558. Hydro frac stimulation was completed with no issues or problems. A separate well visitation report is filed in RBDMS.

Fittings and connections at the well and battery were secure and free from leakage or problems. The tank lids were closed. The secondary containment was intact and dry. The individual enclosure gates were locked. Production reports were current and up-to-date, but the well was not producing at the time of my inspection. The dedicated portion of the shared-use access road was in good condition with some wash-out forming that may require some mitigation. I walked and inspected the areas surrounding the wellhead and battery and found the natural greenscape to be free from signs of impact or degradation. I notified Gonzoil about the possible need for maintenance at the acess road.

The wellhead is comprised of a pump jack unit in a chain-link enclosure. The enclosure gate is locked and secure. Fittings at the wellhead are secure and free from leaks or other issues. There is gas being vented from the 8.625-inch surface casing which should be captured by the system. The load-lines require locks on the valves. I notified the owner about the aforementioned issues and they indicated these items will be resolved immediately.

Our purpose was to observe the stimulation procedure for this well. A stimulation report has been filed in RBDMS. The well and its components are not installed for service at this time. A follow up inspection will be conducted in approximately 3 weeks.
1354816725 2012-12-03 Plug / Plug Back No Violations The visit to the well today was for the purpose if plugging the well back to the Clinton from the Trempealeau formation. A cement report has been recorded in RBDMS. An updated and corrected permit was issued through permitting.

At the time of this inspection, I observed the following: a pump-jack system in a locked and secure chain-link enclosure. Fittings and connections at the wellhead were secure and free from leakage. During a previous inspection, I had noted that gas vented from the surface casing nipple should be plumbed back into the system. That condition had been corrected. Connections and fittings at the battery were secure. The load-lines were locked, which had been noted as a deficiency previously. Tank lids were closed and sealed. The secondary containment was intact and free from signs of leakage or other deficiencies. The dedicated portion of the access road which travels north from the terminus of the driveway was noted to be in good condition. No noted deficiencies at the time of this inspection.

At the time of this inspection, I observed the following: a wellhead with pump jack system and associated tank battery in separate chain-link enclosures located at the back portion of a residential parcel. There is also a indirect heating unit in a board-on-board enclosure located 50 feet north of the wellhead. I found the enclosure gates to be locked and secure, with the following exception: the enclosure gate for the heating unit was broken and not secure. I notified Doug Gonzales about the maintenance needs at the heating enclosure gate. I found the load-line valves to be locked and secure. The tank lids were closed and sealed. The secondary containment was intact and dry. Fittings and connections at the wellhead and battery were secure and free from signs of leakage or problems. I walked the areas around the wellhead and battery which consisted of mixed areas of grass and wooded areas. I did not note any signs of issues or environmental degradation resulting from the on-going production activity.
