The oil release was detailed in Compliance Notice # 1498229525. My inspection revealed that approximately 75 % of the oil had been pumped back into the tank. Mr. Coe had placed straw in the areas oil stained in order to stabilize the oil. Plan forward, as agreed to by all parties, 1.) Remove four (4) tanks that were either leaking or idle, 2.) Remove two (2) concrete in ground brine vaults and dispose of off site ( manifests to be supplied to DOGRM Inspectors, 3.) Remove and properly dispose of all contaminated material, debris and straw, 4.) Replace leaking inspection hatches that were leaking, cut and keep trimmed all vegetation in the tank battery. | I conducted a follow up inspection of the Syak # 1 ( 34-007-2-3188 ) to assess the level of compliance to the violations and remedies detailed in Notice of Compliance # 1498229525. I spoke with the landowner, Robert Munz, who said,  he hasnt seen Mr. Coe for several months. My inspection revealed that none of the prescribed remedies had been performed. The two (2) 210 barrel tanks and partial concrete brine vault were still positioned along the driveway and partially out in the agricultural field. One of the five (5) 210 barrel tanks, that comprise the tank battery, still rests on its side. The gas meters did not have any gas measurement charts. The two (2) remaining buried concrete brine vaults still have not been removed. There remains non-essential equipment on location. The contaminated straw was not removed. I have not seen any progress from the Well Owner / Operator in meeting the items detailed in Compliance Notice # 1498229525. The Notice of Compliance remains in effect.

| I conducted a follow up inspection to assess the level of compliance of the violation noted in the Notice of Violation ( # 1498229525 ) dated 6/21/2017. My inspection revealed that compliance was not  met by the deadline given. There were two (2) 210 barrel stock tanks ( one upright and one on its side ), and an old concrete brine vault lying along side of the access drive and partially out into an agricultural field. These tanks were outside of containment and not essential in the production of the well. I inspected the bottom of the horizontal tank and identified various holes in the steel. There was oil draining out of one of the holes. The tank battery had five (5)  210 barrel stock tanks, five 2408 horizontal separators and two (2) buried concrete brine vaults. There were also two (2) vertical separators in the tank battery and connected to the sales lines. The meters on the 2408 separators did not have any paper charts for gas measurement. Several of the inspection hatches on the 210 bbl stock tanks require replacemnent. There was one (1) 210 barrel stock tank laying on its side in the tank battery which appeared to be partially coated with primer. The remediation of the original oil spill was partially completed. The tainted straw still needed removed and disposed. From my last inspection report I did not see any progress in an effort to bring this well and tank battery into compliance. The Notice of Violation remains in effect.

| My inspection revealed that four (4) 210 barrel stock tanks had been moved to a staging area for removal. One (1) concrete ground vault had been removed and moved to the staging area  awaiting disposal. The contaminated straw had been removed and piled on a trailer for disposal. A 210 barrel stock was lying on its side in order that the bottom to be coated against corrosion. The tank for the Munz # 1& # 2 wells still had the old leaking inspection hatch as Mr. Coe said,  that there was four (4) feet of paraffin and bottom sediment solidified in the tank. He would need to treat with hot water prior to  hatch replacement. Mr. Coe said,  that he was waiting for his contractor to return to remove the tanks. The area where the unused tanks previously sat did not show any contamination. There is additional work to be completed prior to this facility becoming compliant. This work includes, but not limited to, removal of the 210 barrel tanks, excavation and disposal of the two (2) remaining  in ground brine vaults, replacement of the inspection hatch on the Munz # 1 & # 2 tank, rebuilding the containment dike on the north side of the tank battery, removal of all unnecessary equipment from the tank battery footprint, grade, seed and establish vegetative growth. A time extension for compliance has been granted 11/1/2017.

| I inspected the Syak # 1 tank battery ( common tank battery containing six (6) tanks totaling 1,260 barrels ) and found that the tank associated with the Syak # 1 had previously overflowed crude oil that was contained in the earthen dike. The oil was floating on approximately six (6) inches of water from recent rains. The area of contamination measured approximately 90 long X 3 wide and .5 thick of crude oil. The Syak #  tank had oil dripping from the cat walk and on the front of the tank. An additional area of contamination was found west of the tanks surrounding the in ground brine tanks approximating a circle 30 feet in diameter. I contacted Jamie Coe ( Operator ) to inform him of my findings and he replied,  he was aware of it and working on cleaning it up . He alleges that turkey hunters disturbed the setting on the well head which allowed the well to remain on and to flow, breaching the tank capacity. A small gas powered pump was on location to suck up the oil and place it in an empty 210 barrel tank. I asked if he had made the required notification call and the reply was no. I gave Mr. Coe the 24/7  Incident Notification contact number and requested that he call immediately. The remediation of the tank battery shall include dismantling of the tank battery, removal and disposal ( manifests supplied to DOGRM ) of contaminated materials, replacement of soil and pad materials. The remediation shall be completed by 7/7/2017. Operator shall pursue clean up activities continuously until DOGRM grants approval.

I will continue to try and contact Mr. Coe. The Compliance Notice remains in force.
