+ | | I inspected this location as part of an on-going investigation into the source of stray gas impacting the referenced home. A review of historical documents and maps suggests that a former Ohio Fuel Gas well may be located on this parcel. The home owner has reported that there has been a small dead spot near the front garden area for several years. A screening evaluation was performed with punch bar and hand-held gas meter which indicated elevated levels of gas at the dead spot. Based on these findings, the homeowner has contracted with Moores Well Service to perform an exploratory excavation at that spot with a small tracked excavator. A well was discovered at this location. The well is comprised of a 10-inch casing, which is open to atmosphere. The top of the casing is ragged and severely corroded with irregular edges and partially collapsed. Water can be observed within the casing at a depth of 10 feet. The surface of the water is actively bubbling with gas. No other casings are visible. The casing is open to a depth of at least 400 feet based on depth-finder measurements. There is some blackened and stained soils in close proximity to the well but there is no visible oil and oil is not present of the surface of the water inside the casing. Following collection of measurements and other documentation, a passive vent structure was constructed at the well using a plastic 30-gallon container and 2-inch diameter PVC for a vent stack which was affixed to the northeast corner of the house. The well was then re-buried pending further action. I will assist the homeowner in preparing an application for entry to the Idle and Orphan Well Program. | ||

His reported combustible gas readings of 200 to 300 ppm sustained inside the house, with an occasional peak of 600 ppm. He reported that the higher readings tended to be closer to the front of the house, nearer the well. Until yesterday, Mike reported that he had not been seeing any detectable levels of gas. | ||

+ | | I met with the homeowner (Mike Distin) per his complaint of observed low-levels of volatile organic constituents (VOCs) in ambient air, as measured from a hand-held portable gas metering device that he owns. The gas well that had been discovered in the front yard area previously, had now been plugged and abandoned under orphan well program contract. The P&A activity was completed in 2017. Since the completion of the plugging, the homeowner has reported observing levels of detected VOCs in the house, at various concentrations ranging from 20 parts per million (ppm) to 200 ppm. Subsequent to the plugging of the well, and in response to the observed VOCs, the homeowner had a sub-slab de-vaporization system installed by a contractor (A to Z Solutions). Since the installation of the system, the homeowner has reported that the observed levels of VOCs has not changed appreciably. During my inspection, I evaluated the area around the well, the well passive vent stack test port, and the house interior, and found no detectable gases (combustible gas LEL, CO, methane). I did not detect any gas smell or other unusual odors. Crews form Columbia Gas were scheduled to perform an assessment of the domestic supply to the house on the day of my inspection. I advised the homeowner that I would check back following the assessment by Columbia Gas. | ||

At this point, penetration rate is 4 inches per hour. Have blown hydraulic line on swivel. Line repaired. Service Master requesting rig shut down for one (1) hour while they make adjustments to tarps on the house. Rig crew retightening lines and repairing a few small leaks. Drill/cleanout resumes. RW Tree service on location to collect aerial photos of site. Rig has shut down, and will resume drill/cleanout in morning. Estimated depth is +/-180 feet. (Did not want to drill through bridge plug this late in day as the possibility of a large pocket of gas is real.) | |
1434042025 | 2015-06-10 | Orphan | No Violations | I inspected this location as part of an on-going investigation into the source of stray gas impacting the referenced home. A review of historical documents and maps suggests that a former Ohio Fuel Gas well may be located on this parcel. The home owner has reported that there has been a small dead spot near the front garden area for several years. A screening evaluation was performed with punch bar and hand-held gas meter which indicated elevated levels of gas at the dead spot. Based on these findings, the homeowner has contracted with Moores Well Service to perform an exploratory excavation at that spot with a small tracked excavator. A well was discovered at this location. The well is comprised of a 10-inch casing, which is open to atmosphere. The top of the casing is ragged and severely corroded with irregular edges and partially collapsed. Water can be observed within the casing at a depth of 10 feet. The surface of the water is actively bubbling with gas. No other casings are visible. The casing is open to a depth of at least 400 feet based on depth-finder measurements. There is some blackened and stained soils in close proximity to the well but there is no visible oil and oil is not present of the surface of the water inside the casing. Following collection of measurements and other documentation, a passive vent structure was constructed at the well using a plastic 30-gallon container and 2-inch diameter PVC for a vent stack which was affixed to the northeast corner of the house. The well was then re-buried pending further action. I will assist the homeowner in preparing an application for entry to the Idle and Orphan Well Program. |

Mike reported that he had started to observe elevated gas readings inside his house over that last couple of days. His reported combustible gas readings of 200 to 300 ppm sustained inside the house, with an occasional peak of 600 ppm. He reported that the higher readings tended to be closer to the front of the house, nearer the well. Until yesterday, Mike reported that he had not been seeing any detectable levels of gas.
I inspected the vent stack and found it to be venting gas. I also performed a general soil probe survey in the vicinity if the well. I detected a reading of 0.5% methane in a soil probe location at the well site. I did not observe detectable gas readings at any other locations. I advised Mike to contact the FD and seek their consult regarding indoor safety and confirm the gas readings he was observing. Olmsted Falls Fire Chief Munteanu and his assistant arrived and collected gas readings inside the residence which correlated well with Mike Distins readings. At the time of this inspection, combustible gas readings inside the residence are sustained around 230 ppm. I did not detect %LEL or % methane readings inside the house. Chief Munteanu advised Mike Distin to contact Columbia Gas to rule out line gas as a source. Otherwise, the FD did not deem the levels of gas to be an immediate danger. Mike is waiting to see if I&O can provide assistance. |

1662592985 | 2018-03-08 | Orphan | No Violations | I met with the homeowner (Mike Distin) per his complaint of observed low-levels of volatile organic constituents (VOCs) in ambient air, as measured from a hand-held portable gas metering device that he owns. The gas well that had been discovered in the front yard area previously, had now been plugged and abandoned under orphan well program contract. The P&A activity was completed in 2017. Since the completion of the plugging, the homeowner has reported observing levels of detected VOCs in the house, at various concentrations ranging from 20 parts per million (ppm) to 200 ppm. Subsequent to the plugging of the well, and in response to the observed VOCs, the homeowner had a sub-slab de-vaporization system installed by a contractor (A to Z Solutions). Since the installation of the system, the homeowner has reported that the observed levels of VOCs has not changed appreciably. During my inspection, I evaluated the area around the well, the well passive vent stack test port, and the house interior, and found no detectable gases (combustible gas LEL, CO, methane). I did not detect any gas smell or other unusual odors. Crews form Columbia Gas were scheduled to perform an assessment of the domestic supply to the house on the day of my inspection. I advised the homeowner that I would check back following the assessment by Columbia Gas. |

Ran and set a 6.90 inch diameter bridge plug at 180 feet. Northeast Pipe and Supply arrived with 9 joints (+/- Petro Set rigged up and pumping. Plug down after pumping 60 sacks Class A cement with 2% Calcium Chloride. Good cement to surface. |
