| I found the well in production on the day of my inspection.  The well is equipped with a plunger lift assembly.  The gate valve on the tubing is open.  The identification sign is posted at the well.  The fencing around the well was waived during the pre---site inspection due to the location.

| I completed an Urban Preliminary Restoration inspection.  My inspection found the tank battery surrounded by a chain link fence, the proper identification posted, a lock on the fence gate, a dike surrounded the production equipment, and the proper spacing between the 210 barrel oil storage tank, 50 barrel brine storage tank, and the separator.  The well was located in the woods, west of the tank battery, and had the proper identification.  The site was graded and seed to prevent substantial erosion and sedimentation.  Inspection passed.

 +
| At the time of this inspection, the well was equipped with a plunger lift assembly.  Valves on the annulus and flow lines were open.  An identification sign was posted at the well.  The fencing around the well was waived during the pre-site inspection.  The diked tank battery has two storage tanks and a separator.  The pick-up lines have plugs, but no locks.  The 210 BBL storage tank has the required urban safety equipment.  A locked chain link fence surrounds the tank battery.  The identification at the tank battery needs the required information added per 1501:9-9-05(E)(1)(a-e).  The access road was stable with a locked gate, but no identification sign.  Landscaping was waived during the pre-site inspection due to the location.
 +

 +
PAC needs to:
 +
1)  Correct the identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with:
 +
Well owners address and phone number
 +
Property street address or nearest address to the access road entrance.
 +
2) Add signage at the gate access per 1501:9-9-05(E)(1)(a-e).

| I conducted a compliance notice follow-up for compliance notice 547634789, regarding identification installed at the access gate and amanded identification needed at the tank battery per 1501:9-9-05(E)(1)(a-e).  At the time of this inspection, the tank battery identification has been amended with the information as required per 1501:9-9-05(E)(1)(a-e).  The access road gate has had identification installed per 1501:9-9-05(E)(1)(a-e).  Compliance notice closed.

| My inspection found the site graded and vegetation growing.  The pits were filled and all debris removed.  The well was equipped with a plunger lift unit.  The tank battery contained one, 210 barrel production tank, one, 50 barrel brine tank, and a separator.  It was surrounded by a locked chain link fence.  A proper dike was installed around the tank battery and capable of meeting containment requirements.  The proper urban safety equipment was installed on the production tank.  There were identification signs at the well and tank battery.  The lease road was in good, stable condition with crushed limestone.

The equipment on the site consists of the following:

0279569663 2012-09-05 UrbanPreliminary Restoration No Violations I completed an Urban Preliminary Restoration inspection.
0547634789 2018-05-14 UrbanProduction Wells Violations Noted At the time of this inspection, the well was equipped with a plunger lift assembly.

PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Add signage at the gate access per 1501:9-9-05(E)(1)(a-e).

0580449870 2019-03-08 UrbanProduction Wells No Violations I conducted a compliance notice follow-up for compliance notice 547634789, regarding identification installed at the access gate and amanded identification needed at the tank battery per 1501:9-9-05(E)(1)(a-e). At the time of this inspection, the tank battery identification has been amended with the information as required per 1501:9-9-05(E)(1)(a-e). The access road gate has had identification installed per 1501:9-9-05(E)(1)(a-e). Compliance notice closed.
1363277292 2013-03-05 UrbanPreliminary Restoration No Violations My inspection found the site graded and vegetation growing.
