| Operator was on site attempting to cement casing in well to protect ground water and allow for control of well during clean out of well. The operator ran 92 feet of 5 inch PVC pipe into well. The 5 inch pipe was set inside the 6 inch pipe which was located 80-90 feet below ground. Over the past 3 days at different times the operator has put 15 sacks of cement, 30 sacks of cement and has twice poured 3 yards of 9 sack grout into the well in an atttempt to secure the 5 inch pipe. It appears as though there is a void in the ground taking the cement. The operator indicated that there was a bridge in the well at 210 feet depth. Clear water was displaced from the well when 3 yards of 9 sack grout gravity fed into well today. It appears that the 5 inch pipe is beginning to be secured however cement has not yet circulated to surface. Additional cement will be poured into annular space tomorrow in hopes of securing pipe before beginning clean out of well. Inspector Sam Faust also on site.

| Operator mixed bag cement to complete the cementing of the annular space between the well and 5 inch plastic casing last week (refer to inspection report 5/21/08) . This allowed the controlhead to be attached to well to control fluids in well. Well was cleaned out to depth 329 where metal was encountered and no further progress was made. Well plugged at that depth.
