| Operator Tony Krieder on site at McKee well #2. Brush and debris removed from around well head. Rods being removed from well. Pit being excavated for fluids. It appears as though the 6 inch outer casing is cemented into the well. Tony will finish removing rods and then remove tubing. Well will be circulated and bottom plug pumped into well. The 4 1/2 inch casing will then be removed (if possible) and the top plug pumped into well.

| Old rods and tubing removed from well. Well cleaned out to depth of 1280 feet. Ran 2 inch tubing into well inside 4  inch casing and circulated fluids to tanks. Pumped 50 sacks of cement through 2 inch tubing for bottom plug and circulated fluids to tanks. Well produced oil and gas during circulation. Removed 2 inch tubing from well and will allow cement to set up. After cement sets up the well will be circulated again to try to remove remaining oil from well. Operator will then attempt to remove the 4 inch casing before pumping top cement plug.

| Cement set up at depth of 1000 feet from 5 yards of cement pumped on 7/23/09. Operator pulled entire string of 4 inch pipe from well (approximately 500 feet) on 7/24/09 and then pumped 8 yards of class A cement into well. Well did not circulate fluids to surface during this plugging. ODNR MRM inspector Sam Faust witnessed cementing of well on 7/24/09. Cement set up over weekend, then on 7/27/09 the top of cement was located at depth of 500 feet. Since well did not circulate fluids on 7/24/09, some of  the cement went into the rock formation. The 13 yards of cement pumped into the well over the 2 days should have been sufficient to plug the well. Rather than pump more Class A cement, the decision was made to reverse grout the well with 9 sack grout. Cement grout  was gravity fed into the well on the annular side of 500 feet of 2 inch tubing. Water was displaced through the 2 inch tubing and circulated to surface.
