| I received a call from Mr. Fox(330-398-9065) about getting a water well sample. I stated that he was mad about all the phone messages telling him when the water sample crew would be out to take his water well sample. I told him to call me when he and his wife have a time when the water sample is to be taken. I first said that anytime next week or the week after Christmas. He would call me either tomorrow of Monday, 12/20/10 about the time. I called John Howell Tool Pusher on(330-301-0769) Wildcat rig #1. He said they would be moving to this location at the earliest by next Tuesday. The rig would shut down for Christmas on 12/22/10. I felt that I was wrong on what I told Mr. Fox. I called Mr. Fox back and told him that the water well samples needed to be taken either Monday or Tuesday, 12/20/10 or 12/21/10. He thanked me for calling. Steve Opriza had a phone conversation with Mr. Fox and told him to call Everflow to get a date and time to have his water well sampled. Mr. Fox stated on the phone to me that he had not called Everflow. The letters and phone messages from Everflow were not nice as told to me by Mr. Fox. Kyle tested the well and preliminary reports shows no sign of a oil water well problem. The water well had bacteria in it. I called Mr. Fox back and told him that the water well samples needed to be taken either Monday or Tuesday, 12/20/10 or 12/21/10. He thanked me for calling. Steve Opriza had a phone conversation with Mr. Fox and told him to call Everflow to get a date and time to have his water well sampled. Mr. Fox stated on the phone to me that he had not called Everflow. The letters and phone messages from Everflow were not nice as told to me by Mr. Fox. Everflow had called numerous times to set up a date and time for taking the water sample. No response from Mr. Fox. No fence around well head. Tank battery in place. Dike constructed with gravel bottom. One 70 bbl stock tank, one poly brine tank, one separator. Flow line laid out. No fence or ID sign on tank battery. Well is not in production. Weather conditions delaying progress of restoration. I called Ms. Hawkins (330-853-5461) on 06/27/2011 at 16:05 to report the findings of my inspection. Complaint resolved. No violations at this time. |
