| Shane Speedy Drilling spupped a 8 7/8 hole 520 deep and ran 512 of 7 20 #  J-55 casing. Nabors Complection and Production Services broke circulation and pumped 500 lbs of Gel followed by 105 sacks of class A cement with 3% CC. displaced with 21.4 barrels of fresh water landed plug with 520 psi. We had 8 barrels of cement returned to surface. Plug would not hold, pressured casing back to 520 psi shut well in at the cement head will remove on 7/18/2013.

| I performed a status check on this location. All drilling fluids have been removed and disposed of from the working pit. The pit has been solidified and covered with soil. The location is rough graded due to the rain and wet weather, final grading will be completed when location dryes out. This is a Urban Area Drilling location.

| I was onsite to check the status of the cable tool rig drilling this Berea sandstone well. Currently the well was around 245' and drilling around 6-8 feet per hour. The rig hit some coal while drill near 235' but is now through the coal seam and drilling through shale. There will be a bulldozer coming on location to move some equipment around and improve the drainage of the site. There are no violations at this time.

| I was onsite to check the status of this cable tool rig Berea sandstone well being drilled. The rig was about to be done for the day drilling and was drilled around 140 feet at the time. They stated drilling will continue next week and I may be several weeks before the rig would be cementing surface casing. I will check back at another time to check the status of the drilling. There are no violations at this time.

| Speedy Drilling drilled a 8 7/8 hole to a total dept of 520' and ran 512' of 7 20# J-55 casing. Nabors Completion & Production Services pumped 3.5 barrels of fresh water and established circulation. They pumped 105 sacks of class A cement and displaced with 21.4 barrels of fresh water with 8 barrels of cement returned to surface. Pumped plug to 520 psi would not hold, bumped plug a second time still would not hold, shut cement head in will wait 24 hours and check. After 24 hours fluid stayed at the top of the cement head and had dropped about 8 around the conductor ( 9 5/8 ).
