| My inspection found the well to be equipped with a pump jack unit. The well and tank battery are surrounded by a chain link fence. The tank battery consists of a 210 BBL production tank and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition.

| I was notified by Ken Folsom of Geauga County Emergency Services of a well leaking gas and crude oil. I arrived on location to find the Middlefield Fire Department and Jim Stanley from Petrox working to clean up the oil. There appears to be about 5 BBL's of oil that had escaped from a flow line that had rusted through. The well had been shut in and the line was not flowing any more oil from it. The crew used absorbent mats to clean up the oil. In the morning petrox will have the oil saturated soil removed and replace it with new soil.
