At the time of this inspection, I observed the following: Ingress and egress from the well area is afforded by the shared asphalt-paved driveway and parking areas for a multi-tenant retail strip shopping area that fronts Mayfield Road. A wellhead with mechanical plunger lift system is located in a board-on-board enclosure in an otherwise open area in the rear parking lot. The well enclosure was locked and secure. Fittings and connections at the wellhead were secure and free from leakage. The 8.625-inch casing annulus was exposed and plumbed into the system. Areas around the wellhead consist of asphalt parking lot. The tank battery is located 300 feet southeast of the wellhead in a grassy fringe area. The battery is comprised of a 100-barrel, low-boy steel production tank, one 50-barrel poly brine tank. At the time of this inspection, the access gate to the battery was locked and secure. Signage was present. Fittings and connections were secure and free from signs of leakage or problems. Process safety gear and carbon filter system was present. Secondary containment was intact and dry. Tank lids are closed. The load-outs were plugged and locked. Evergreen screening is present surrounding the battery and was good condition. I walked the area surrounding the battery and observed normal grassy vegetation with no signs of erosional deficits or impacts associated with the production operation.
0709260089 2017-04-04 UrbanProduction Wells No Violations I inspected this well location pursuant to complaint (#5575) regarding heaving of asphalt pavement at the the well. Upon my inspection, I noted two, large linear areas of heaved asphalt next to the wellhead, co-incident with the former locations of the drilling pits. The vertical offset was approximately 3 to 6 inches. I contacted Brooks Yarborough about the need for mitigation. Brooks indicated that BAAY Oil was aware of the problem and would remedy within the next 30 days. I contacted the complainant and advised that BAAY OIL was going to remedy the problem.

The wellhead consists of a mechanical lift unit and is producing. The annular space is exposed and gauged, showing 60 PSI. The tank battery is comprised of 2 low-boy 100 barrel tanks and a 25 barrel poly tank. HB 278 equipment is in place and the secondary containment is good. The interior spacing is in accordance with the rules. There is also a Dominion metering run in the battery. I did not observe any problems or violations. The unit passes preliminary restoration.

The well and battery are located behind the main stretch of retail buildings along Mayfield Road. Access to the unit is via the parking lot of the restaurant at 5900 Mayfield Rd. Connections and fittings at the wellhead and battery are secure. Enclosure gates are locked. Signage is present and accurate. Load-outs are plugged and secure. Production reports are current. At the time of this inspection there is 12 to 15 inches of snow cover, which obscures some of the surficial features. The areas surrounding the wellhead and battery are comprised of a grassy tree lawn adjacent to the parking area. The tree lawn is snow-covered. I did not observe any signs of impacts relating to the well on the surrounding areas.
