Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0590074792 2014-03-25 Production Wells No Violations Status check inspection found the following equipment present at the well head- wooden conductor- 7 casing-4.5 casing- 2.375 tubing and pumping rods with the production valves open and the well set up to be produced by the means of a pumping unit, at the production fluid storage area a single 100 barrel tank is present along with a 60 barrel production tank.
0927605734 2019-02-26 Production Wells Violations Noted On this day, I found the well equipped with 7 inch diameter surface casing, 4.5 inch diameter production casing, and 2.375 inch diameter tubing, with flowlines attached to the production casing and tubing with valves open. It is setup to be produced by means of a pumping unit with rods and an electric motor. There is (1) 100-barrel steel tank with a knock-out separator on top, and (1) approximately 70-barrel steel tank associated with multiple wells on the property. No identification is present at the wellhead or the tank. According to 1501:9-9-05(A)10, legible identification shall be posted in a conspicuous place on or near the wellhead or the storage tank(s) as to owner, lease name, well number, permit number, county, and an emergency contact number. If multiple wells are being produced into the common tank(s) each wellhead shall be identified as previously stated. Production records are current through 2016. Multiple pictures were taken.
1469182847 2016-07-21 Production Wells No Violations On site status check inspection found a service rig on the hole belonging to Chuck Dye and the workers preparing to pull the pumping rods and replace the downhole pump. The service rig owner Chuck Dye was on site and we discussed his need to line any pits that are dug and used in any well repair or plugging process, it was explained that while the Mississippian Exemption clause did exist in the law, no pits used in the workover or plugging process could have their contents drained onto the Earths surface or into the waters of the State. Mr. Dye was also informed of his need to obtain a operating bond and register his wells with the State. Also noted is the registered owner of the well Walter Dye has passed away and no production has been reported to the Division since 2013 for any of the wells registered under owner 1281.
