1097431279 | 2019-04-04 | Production Wells | No Violations | On this day, I am accompanied by ODNR Inspector Rick Brown. I found the pad gated with signage at the entrance. There are (3) producing wells, (3) gas processing units, (2) 400-barrel steel production tanks within steel containment, and an Emergency Shutdown (ESD) button is present at the pad entrance. Concrete barriers surround the perimeter of the wellheads. Each well has identification and annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 140 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 300 psi. The production records are current. |

1482948156 | 2016-12-28 | Casing String Witnessed | No Violations | I was on location to witness Express Energy cement a 30 inch diameter conductor casing. The 30 inch diameter conductor is 100 feet deep in a 36 inch diameter bore hole. After Express had finished cementing the Pfalzgraf S U1H Express realized they would not have enough water to finish circulating the cement to surface. Express had one of the water truck drivers go and get another 40 barrels of water before we could start cementing this hole. Once the water truck driver returned Express began to prepare the hole for cement. Express ran 12 barrels of fresh water gel ahead of the cement. Express then ran 275 sacks of cement, 2% CaCL, and .50 pounds per sack of floseal with 57.79 barrels of fresh water at 15.6 pounds and a yield of 1.18. Express then circulated with 74.80 barrels of fresh water until we got cement to surface. Express circulated about 10 barrels of cement to surface and finished cementing the conductor into the bore hole. |
1489714838 | 2017-03-16 | Casing String Witnessed | No Violations | On site inspection to witness the surface casing cement job with Well Service Group rig # 1 and Halliburton Cement Service. The rig has drilled a 26 diameter surface borehole to a measured of 481 and ran 20 diameter J- |
1520361506 | 2018-03-06 | Completion Testing | No Violations | Status check inspection found the well drilling and completion process now complete. There is a primary master valve installed on the well head. The well is set up to be produced out of the 5.5 production casing with a production stack assembled on the casing well head consisting of (2) manually controlled valves and (2) emergency shut don valves. Crews are on site installing the production equipment and flow lines. A production fluid facility is being assembled and will consist of a total of (2) steel tanks capable of holding 400 barrels total capacity each. The wells are plumed up to accommodate annular pressure monitoring. The preliminary restoration will need followed up on later in the year once vegetation can be better established. The well pad outward slopes appear to be stable and the well pad surface is of good repair and of stone construction. |
1661707508 | 2017-12-14 | Completion Testing | No Violations | Status check inspection on the well completion process found the well drilling -stimulation and completion process has been completed and crews are on site installing the production well head assembly. The location is also being cleaned up to be prepared to turned over to production. |
