1240398103 2018-02-01 Casing String Witnessed No Violations I was on location today to cover the cement job on the conductor. When they originally cemented the conductor back to the surface they had trouble getting returns. Formation Cementing is back out on location today to run 1 inch plastic down in the well and top it off. When running the 1 inch plastic in the well they sat down at 12 feet. The cement was pumped through the 1 inch plastic until we had good thick returns to the surface. Inspector Mitchell was able to make it out to relieve me.
1517255345 2018-01-29 Drill / Deepen / Reopen No Violations I met with company man John Suhar and Kilbarger Drilling Superintendent Gary Smith on location. Kilbarger was hauling rig equipment in during the inspection. The access road was built with stone. The access road crosses over the T.J. Evans Foundation Bike Path. Stop signs were posted at the bike path for incoming and outgoing traffic during drilling operations. I discussed the drilling program with Mr. Suhar and Mr. Smith. 1517842401 2018-01-31 Casing String Witnessed No Violations I witnessed the cementing of the 11.75 inch conductor casing on Kilbarger rig #5 for David R. Hill Inc. John Suhar was on location for David R. Hill Inc. Formation Cementing was contracted to cement the casing. Formation achieved circulation but cement returns were minimal. It was decided to pump cement with a tremie pipe on the 8 inch cement job. Refer to cement job report for further details. Time of inspection: 12:00 PM.
1518042325 2018-02-06 Drill / Deepen / Reopen No Violations My inspection of drilling operations revealed that the rig was drilling on mud at a depth of 3830 feet. The rig reached a depth of 3861 during my inspection. The mud loggers were picking up gas even though the rig was drilling on mud. David R. Hill determined that the gas was probably coming from just below the Berea in the Big Cinnamon (Ohio Shale). The rig was then put on stand-by until the company decided on how they wanted to drill the well in. Time of inspection: 8:55 AM.
1518110994 2018-02-07 Drill / Deepen / Reopen No Violations My inspection revealed that Kilbarger rig #5 was still at a depth of 3860 feet. David R. Hill Inc. decided that they would install 5.5 inch production casing instead of 4.5 inch casing. The casing would be set at the current depth and the well would be drilled in as an open completion next week. The rig was pulling drill pipe and collars during the inspection. 1518795496 2018-02-16 Drill / Deepen / Reopen No Violations I inspected drilling operations today. The rig was not drilling during my inspection. Heavy rain overnight has caused high water levels around the well location and made access to the rig inaccessible in a truck. The water level did not reach the well location. David R. Hill Inc. decided against drilling until the water levels recede for safety reasons. The rig went back to drilling on fluid yesterday morning and the current depth is 3,935 feet. 1519136588 2018-02-20 Drill / Deepen / Reopen No Violations My inspection revealed that Kilbarger rig #5 has reached total depth and drilling operations are complete. Kilbarger drilled in on 2/19/2018 to a depth of 3,975 feet. During the inspection the rig and equipment was being hauled out. The tubing and drill bit was left in the hole. A service rig will move in at the end of the week to pull the 2.88 inch tubing and bit. The service rig will then run 2.38 inch tubing and complete the well. Time of inspection: 8:15 AM. 1519311313 2018-01-31 Drill / Deepen / Reopen No Violations I witnessed the pressure test on the 11.75 inch casing. The test started at 11:45 PM with 400 psi. The pressure dropped approximately 20 psi within 15 minutes. The rig crew tightened up the gauge and fittings on the cement head and we began a new test at 12:00 AM with 380 psi. At 12:30 AM the gauge showed 342 psi. The test passed with a loss in pressure equal to 10 percent. Time of inspection: 11:45 PM.

1531228595 2018-07-10 Preliminary Restoration No Violations My inspection revealed that the well was equipped with a pumping unit and gas motor. The tank battery consists of (1) 210 barrel tank inside an earthen dike. During the inspection Production Supervisor Dan Walker was at the well when I arrive. Mr. Walker told me that the current location of the tank battery is temporary and a new tank battery will be set up closer to Raccoon Valley Rd. The well location has been restored. All drilling equipment has been removed and the pits were closed. Soybeans have been planted up to the well location. The well location meets ORC 1509.072 requirements. I did not observe identification at the well or tank battery. I advised Mr. Walker to post identification during the inspection. Time of inspection: 9:00 AM. Weather: 77 degrees F, Sunny
1593675093 2018-01-30 Drill / Deepen / Reopen Violations Noted I received a phone call from John Suhar at 1:20 PM reporting a diesel spill on the drilling location. I arrived to the drilling location at 1:30 PM and met with toolpusher Gary Smith. The well had not been spudded when I arrived to the rig. Mr. Smith said that his rig crew overfilled a diesel tank when they were pumping diesel from one tank to another. The amount of diesel spilled was estimated to be 200 gallons. The spill did not leave the drilling location. The rig crew was in the process of cleaning up the diesel when I arrived. Two 55 gallon drums of diesel had already been vacuumed into the drums. I contacted Emergency Response Manager Trisha Cartmell and she advised me to have David R. Hill Inc. call the Ohio One-Call System. David Hill and John Suhar arrived at approximately 2:00 PM. A Kilbarger Construction brine hauler truck arrived and began vacuuming diesel. The truck was properly labeled with UIC #25046. A clean-up plan was discussed with toolpusher Gary Smith, David Hill, John Suhar, and myself. It was decided to vacuum as much free product as possible and due to the ground being frozen that the contaminated soil be excavated after the well has been drilled. Kilbarger had a load of oil absorbent delivered to the location. The aborbent would be spread on the contaminated areas once free product was removed. The company shall notify me once they are ready to excavate contaminated soil. Time of inspection: 1:30 PM. Weather: 24*F, Partly cloudy

The drilling rig and equipment has been removed. Kilbarger excavated the diesel contaminated soil on 2/21/2018 and hauled it to the Kilbarger landfill in Logan. 1788173441 2018-05-01 Completion Testing No Violations I witnessed the acid job on this well today. David R. Hill Inc. decided to re-acidize the well. Berry Well Service was contracted for completion services. Producers Service Corporation was contracted to pump acid. Berry Well Service ran 2.38 inch tubing on an open hole packer into the well. The packer was set at approximately 3,939 feet. The interval that would have been treated was from 3,939 feet to 3,975 feet. David Hill and John Suhar were present for the acid job. Time of inspection: 8:30 AM.
1861210254 2018-02-01 Casing String Witnessed No Violations I witnessed the cementing of the 8.63 inch surface casing on Kilbarger rig #5 for David R. Hill Inc. John Suhar and David Hill were present for the cement job. Formation Cementing was contracted to cement the casing. Kilbarger ran 505 feet of new 24 lb./ft. surface casing. Formation achieved circulation and good cement returns were observed. Cement did not fall back. Prior to the 8 inch cement job Formation Cementing pumped 30 sacks of cement to surface on the backside of the 11 inch casing with a tremie pipe. Refer to cement job report for further details.
