0156836099 2012-09-14 Final Restoration No Violations My inspection found all pits closed, debris removed, the site graded and raked to prevent erosion, and all production equipment removed. The landowner will seed the site and lease road has been waived for personal use.
1345466896 2012-08-03 UrbanProduction Wells No Violations On 8/3/2012, I performed an inspection of the restoration at this site with Matt Kleese of Kleese Development Associates and the landowner. I informed Matt that the site needed to be reclaimed and all ruts repaired after the plug job is complete. The landowners representive, Marty Cisine, told Matt not to reseed because he wanted to reseed with clover and other special vegetation for the wildlife.
1345566087 2012-08-08 Plug / Plug Back No Violations On 8/8/2012, I witnessed a plug job by J Well Service for Kleese Development Associates. The bottom, Big Lime, Surface Casing, and Surface plugs were set. The 4.5 production casing was ripped at 3,952 and a total of 92 joints plus two 12 joints of casing were removed from the well. Please refer to the plugging report for further information.
1729985791 2012-11-28 Final Restoration No Violations I completed a final restoration inspection. My inspection found the well plugged on 8/8/2012 and performed by Kleese Development. The pit was filled and debris removed. All of the production equipment was removed and the site was graded. The landowner was going to spread new grass seed. The landowner wanted the lease road to remain; however, refused to sign the landowner waiver. The gate at the entrance of the lease road was left intact with the landowners chain and lock.
