Shannon Stevens (Company Representative). A retrievable bridge plug was set at 2150. They dumped 40 gallons of sand on top of the bridge plug. Approximately 5 of fill on top of plug. They loaded the hole with 83.5 Barrels water. Tested the well and plug to 700psi. They ran first log with out pressure. Ran second log under 1000psi to 2133.9 they maintained between 900psi and 1000psi with an average psi of 960. I sent copies of the logs to ODNR Columbus office for evaluation.
0567979377 2008-11-17 Preliminary Restoration No Violations Pit was solidified today with 280 sacs of cement brought to location by BJ in a tear drop truck. Dry cement was blown into pit and stirred up with a trac hoe bucket. (Rose Excavation) Jimmy Ray was also on site for the solidification. After pit set up they started backfilling the pit.
0721088339 2008-11-12 Drill / Deepen / Reopen No Violations Service rig on location. Pit has been pumped down. Some fluid in bottom of pit. Liner is in good shape. Pit is to be solidified before closing. The wellhead has been grouted between the 9 5/8 and the 7. The grout job was done on the 10th. I have requested a report and cement tickets for this well to determine where the gas is coming from on the backside of the 7. The gas and brine leak has been stopped at the surface by the grout job. The subsurface gas leak has not been resolved. My gas detector showed no sign of gas at the wellhead.
0917156640 2009-05-13 Production Wells No Violations No identification. Audible gas leak at wellhead between the 7 collar and the 7 by 2 swedge. Gas meter shows no explosive levels of gas. Well is equipped with a 7 by 2 swedge a 2 valve, 2 bull plug with a 1/4 needle valve. There is no fluid around the wellhead. Restoration has not been attempted, site is rutted no seed or mulch has been applied to location. Top soil is in a pile. Pit has been solidified and closed. Took pictures.
1096748001 2008-10-30 Drill / Deepen / Reopen No Violations This site is located in a flood zone. Earthen pits were allowed only during the dry season. The main condition to allow earthen pits was that as soon as drill rig moves off location the pits are to be solidified. Pit is still open. Fluid has been pumped down and 1.5 of the pit divider is visible. Liner is in place and does not appear to be compromised. Wellhead is leaking brine and gas between the 9 5/8 and the 7 casings. The brine is being contained in the cellar at the wellhead. Gas level was 14 LEL on the PhD Lite gas meter. I contacted Shannon Stevens and we went and looked at the leak. We called a water truck to remove the brine from around the wellhead. We were then able to see the leak on the backside of the 7 casing. Merl Richards and Ed Kohler were notified of the leak.

1233164918 2008-12-19 Completion Testing No Violations Small bubbles 1/8 inch were observed at the wellhead. Still waiting on the cement bond log to see if leak has been addressed by the grout job.

1785326454 2009-01-12 Drill / Deepen / Reopen No Violations Retrievable bridge plug set at 2150 with sand on top of it. Running tubing down today to the top of the sand and will circulate the sand out of the hole then retrieve the bridge plug.
