| I met today with landowner, Judy Knorr, regarding this idle well and the status of the progression  on the old Chief's Order.  I had e-mailed attorney Megan De Lisi after being referred to her by Erica Freemen of theDOGRM bonding department and have yet to get a response as to where this case falls in the enforcement issues.  Dan Goins, Supervisor for Washington County, has the Chief's Orders laying on his desk but they are not listed when the M/drive list of Orders is reviewed.  Bob Brenneman has verbly agreed to assign the 2 shallow wells to Judy and she has sent in paperwork to become bonded as a registered operator.  I can get no update on the current status or what is left to do to get this process completed and Judy is frustrated.  This well needs plugged, still is idle with a reported hole in the casing or tubing, and action needs to be taken.  Violations were wrote several years ago that led to the Chief's Orders.  Judy is asking what she can do to get this well plugged

| The wells location is still overgrown with weeds, multiflora roses and a small tree. I had to beat down the weeds to be able to get pictures of the wellhead. There was a gate valve, and I was not able to determine whether or not it was open. There was another valve on the production line that was in the open position near the wellhead. There is no evidence of activity near the wellhead. There are several pieces of steel flowline laying near the dike not connected to anything. There was an elbow that seemingly went to the separator that had reduction nipples in it that then had a plug in it. This would appear to have went from this well to the separator. The motor valve near the separator in the dike area was inoperable. There were no batteries in the unit and it was not on. The dike also contained a 40-barrel plastic tank, and a 100--403. Pictures were taken at the time of this inspection.

| I received call from Bob Brennaman's attorney agreeing that lease had been cancelled, questioning what could be done to resolve issues and could they have an extension.  He said he would talk to landowner's attorney and get back with me.  I will hold off turning in C.O.'s request for a couple weeks - has been completed, having worked on it several hours today and yesterday.

| I received a voice message from Bob Brenneman at 4:45 PM this day, stating that he had got MY voice message and also received Certified paperwork from us over the weekend.  I am unsure what paperwork he is talking about because NOV was issued 6/09.  He says he has contacted legal counsel and also reports, in the voice message, that Ergon has informed him that they picked up a tank of oil for which he has not been paid, believes the landowner has stolen his oil and feels that holds the lease for another year.  Not sure where he is getting his information

I was unable to locate a valve on the single connected production line on the well but, the well appears to be idle and no production has been reported for this well since 2003. The area around the wellhead is overgrown with brush and trees and the well appears to not have been worked on in some time. The tank battery for this well is shared between this well and the other two wells on the lease permit number 27777 and 27778 and there is no identification on the production equipment for any of the three wells or identification on the wellheads themselves. The production equipment for this well consists of a 100 barrel steel tank, a vertical separator, a plastic tank that has grass growing in it, and a meter box with a chart that is not current in it. There has been no resolution to the issues on this well and the well is idle at the time of inspection.
0528828886 2018-08-07 Production Wells No Violations I am on location today to do a follow-up inspection on this well. The well at the time of my inspection appears to be still sitting idle and incapable of production with no change in the status of it. The vegetation around the well has completely covered the well showing the well has not been in operation in some time. The well has reported zero for production since 2003. The company was issued a compliance notice in 2009 to plug the well for non production but has yet to do so. Plugging of the well is still required at this time. I have taken updated photos of the well for my inspection report.

1136617505 2012-10-31 Administrative Inspection No Violations I called Bob Brenneman at Mayors office in Wooster and left a message to call me. I have tried repeatedly to contact him at the Ponderosa Consultants phone number and get same message over last 5 years - mailbox full. The lease has been cancelled and recorded in the Washington Co courthouse Vol 532 OR, page 2111 and Bob has only the option to plug well. Bob returned my call at 4:45 PM and left a message indicating he still owned the wells and seemed unaware that lease had been cancelled. Supervisors suggested I send out Certified copies of NOVs to all 3 addresses, including the Mayors office.

Gave 1 well NOV but not all 3. Too many NOVs now.

The landowner wants to keep the 2 Berea wells and this well will need to be plugged by Ponderosa - I asked a time frame be put on that plugging. Alice called from Cambridge to say the 2 undelivered Certified letters have come back to our office with the 3rd letter being delivered. Asked her to forward those to New Philly to go with other documents for this lease. Will hold off on the Chiefs Orders to see what the attorneys get worked out.

1368798133 2013-05-16 Administrative Inspection No Violations Judy Knorr, landowner, called to say Ponderosa has not plugged well or picked up oil. She said the deadline for him to do it is 5/30/13. I suggested she wait to see if he gets it, then contact lawyer after that date to see what he suggests. I would like to see oil picked up as full tank that has been sitting there for several years. She does not have 2 Berea wells in her name yet and told her the process for putting them in her name. Ponderosa probably will not sign a Form 7 and she may have to send paperwork to Columbus showing she is the legal owner. It will be up to Columbus then - lawyer is helping her. She will need a $10,000 and no insurance since she is landowner.

I returned his call this day after getting the message, but again missed talking to him - left another message. Landowner, Judy Knorr, has cancelled the lease as of 10/9/12 and he seems also unaware of this.

1533303740 2018-08-03 Production Wells No Violations The wells location is still overgrown with weeds, multiflora roses and a small tree. I had to beat down the weeds to be able to get pictures of the wellhead. There was a gate valve, and I was not able to determine whether or not it was open. There was another valve on the production line that was in the open position near the wellhead. There is no evidence of activity near the wellhead. There are several pieces of steel flowline laying near the dike not connected to anything. There was an elbow that seemingly went to the separator that had reduction nipples in it that then had a plug in it. This would appear to have went from this well to the separator. The motor valve near the separator in the dike area was inoperable. There were no batteries in the unit and it was not on. The dike also contained a 40-barrel plastic tank, and a 100--403. Pictures were taken at the time of this inspection.

The dike where the production equipment is located is filled with water to a depth of at least a foot in deeper areas on the day of my inspection. The chart in the meter was not current. Well and production equipment is located in a pasture field. Nothing has been done to comply with the Chiefs Orders. Pictures were taken to assist this inspection.

1908158888 2012-11-07 Administrative Inspection No Violations Called again this morning from home phone to contact Bob Brenneman at Wooster Mayors office to discuss his NOVs and resolution. Left a message and will wait for a reply. Bob called late today to say he still owns lease - he did not get notice that lease cancelled and I gave him Vol, Page where he could find it at Washington Co courthouse. A field inspection done this day for final inspections, pictures to begin Chiefs Orders. This well not producing, valves closed, and too much water inside dike area to check meter box - no outlet for gas so I know there is no chart. Altho Bob has had 9 years to get the wells in production, with a yearly call, if not many more, plus a discussion with Rob Stonerock, Supervisor several years ago, Bob now thinks he can have wells back in production and I verbally gave him 1-2 months. He says 2 full tanks of oil but no load line to sell it - that will be the 1st thing he does, after contacting attorney to investigate claim that lease no longer valid. My research shows lease not valid and he will need to plug wells.
