| Inspector James Bucksar and I were on site to witness the cementing of the 13 3/8 inch surface casing. Our inspection revealed that Nomac Drilling Rig # 71 has drilled a 17 1/2 inch hole to a total depth of 575 feet and ran 565 feet of 13 3/8 inch surface casing (54.5 pounds per foot / inside diameter is 12.615 inches). They installed 7 bowspring centralizers on the casing string on joints (2 on 1st, 2,3,6,9, and 12). Schlumberger cemented the casing with 514 sacks of Class A D901 with 1% D154, 0.5 B348, 0.15% B160A,, 0.2% D046, .125 pound per sack D130. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.219 gallons of mix water per sack. Schlumberger pumped 110 barrels of cement slurry, 80.5 barrels of displacement, and there was 21 barrels of cement returns to surface. They landed the plug with 275 PSI and bumped the pressure up to 728 PSI for the casing test. The casing test was approved, they released the pressure, the floats held, and there was 1/2 barrel of water flowed back when the pressure was released. Our inspection revealed no violations at this time.

| I was on site to witness the cementing of the 9 5/8 inch intermediate casing. My inspection revealed that Nomac Drilling Rig #71 has drilled a 12 1/4 inch hole to a total depth of 1896 feet and ran 1888 feet of 9 5/8 inch intermediate casing (40 pounds per foot / inside diameter 8.830 inches). They installed 17 bowspring centralizers on the casing string; 2 on 1st joint,2,3,6,9,12,15,18,21,24,27,30,33,36,39,and 42. O-TEX cemented the casing with 690 sacks of Class H with 3% Gypsum, 2% CaCl, 0.8% MC500,0.5% C45, and 0.4% C-41P. The cement had a density of 15.8 pounds per gallon, a yield of 1.21 cubic feet per sack, and they used 5.14 gallons of mix water per sack. O-TEX pumped 149 barrels of cement slurry, 133 barrels of displacement, and there was 28 barrels of cement returns to surface. They bumped the plug with 1110 PSI and then took the pressure up to 1606 PSI and conducted the casing test. The casing test was approved, pressure was released, floats held, and there was 3/4 barrel of water flowed back. My inspection revealed no violations at this time.

| I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Enviro Drill has drilled a 30 inch hole to a total depth of 120 feet and ran 120 of 20 inch conductor casing (78.67 pounds per foot / inside diameter of 19.25 inches). Allied pumped 433 sacks of Class A cement with 2% CaCl and 1/2 pound per sack floseal. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.23 gallons of mix water per sack. Allied pumped 91 barrels of cement slurry, 39.5 barrels of displacement, and there was 10 barrels of cement returns to surface. My inspection revealed no violations at this time. NOTE: Started pumping water on the 6H well and found the 2 inch ball valve on the conductor cap was split. Enviro Drill replaced the ball valve and Allied started pumping on the casing again. Had to shut down again because the weld on the conductor cap was leaking. They also found that the 5H and 10H conductor holes were communicating so they decided to pump this cement job first and then go back to the 5H and 10H conductor cement jobs. Region Supervisor Tom Hill was present for the cementing of the casing.

| I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Enviro Drill has drilled a 30 inch hole to a total depth of 120 feet and ran 120 of 20 inch conductor casing (78.67 pounds per foot / inside diameter of 19.25 inches). Allied pumped 433 sacks of Class A cement with 2% CaCl and 1/2 pound per sack floseal. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.23 gallons of mix water per sack. Allied pumped 91 barrels of cement slurry, 39.5 barrels of displacement, and there was 10 barrels of cement returns to surface. My inspection revealed no violations at this time. NOTE: Started pumping water on the 6H well and found the 2 inch ball valve on the conductor cap was split. Enviro Drill replaced the ball valve and Allied started pumping on the casing again. Had to shut down again because the weld on the conductor cap was leaking. They also found that the 5H and 10H conductor holes were communicating so they decided to pump this cement job first and then go back to the 5H and 10H conductor cement jobs. Region Supervisor Tom Hill was present for the cementing of the casing.

| Inspector James Bucksar and I were on site to witness the cementing of the 13 3/8 inch surface casing. Our inspection revealed that Nomac Drilling Rig # 71 has drilled a 17 1/2 inch hole to a total depth of 575 feet and ran 565 feet of 13 3/8 inch surface casing (54.5 pounds per foot / inside diameter is 12.615 inches). They installed 7 bowspring centralizers on the casing string on joints (2 on 1st, 2,3,6,9, and 12). Schlumberger cemented the casing with 514 sacks of Class A D901 with 1% D154, 0.5 B348, 0.15% B160A,, 0.2% D046, .125 pound per sack D130. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.219 gallons of mix water per sack. Schlumberger pumped 110 barrels of cement slurry, 80.5 barrels of displacement, and there was 21 barrels of cement returns to surface. They landed the plug with 275 PSI and bumped the pressure up to 728 PSI for the casing test. The casing test was approved, they released the pressure, the floats held, and there was 1/2 barrel of water flowed back when the pressure was released. Our inspection revealed no violations at this time.

| I was on site to witness the cementing of the 9 5/8 inch intermediate casing. My inspection revealed that Nomac Drilling Rig #71 has drilled a 12 1/4 inch hole to a total depth of 1896 feet and ran 1888 feet of 9 5/8 inch intermediate casing (40 pounds per foot / inside diameter 8.830 inches). They installed 17 bowspring centralizers on the casing string; 2 on 1st joint,2,3,6,9,12,15,18,21,24,27,30,33,36,39,and 42. O-TEX cemented the casing with 690 sacks of Class H with 3% Gypsum, 2% CaCl, 0.8% MC500,0.5% C45, and 0.4% C-41P. The cement had a density of 15.8 pounds per gallon, a yield of 1.21 cubic feet per sack, and they used 5.14 gallons of mix water per sack. O-TEX pumped 149 barrels of cement slurry, 133 barrels of displacement, and there was 28 barrels of cement returns to surface. They bumped the plug with 1110 PSI and then took the pressure up to 1606 PSI and conducted the casing test. The casing test was approved, pressure was released, floats held, and there was 3/4 barrel of water flowed back. My inspection revealed no violations at this time.

| I was on location to conduct a preliminary restoration inspection.  The #6H well is in production and is equipped with plunger lift assembly.  Jersey barriers were installed around the wellhead.  The well is identified with a legibly-marked well identification sign.  Identification was posted at the tank battery which consisted of  eight 400 bbl production tanks, four  labeled as containing produced water, and four labelled as containing condensate, and one low-pressure vertical separator.  The tank battery is surrounded by a dike.  There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters.  There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks.  Production equipment on the well pad consisted of:

0332798325 2014-02-03 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Enviro Drill has drilled a 30 inch hole to a total depth of 120 feet and ran 120 of 20 inch conductor casing (78.67 pounds per foot / inside diameter of 19.25 inches). Allied pumped 433 sacks of Class A cement with 2% CaCl and 1/2 pound per sack floseal. The cement had a density of 15.6 pounds per gallon, a yield of 1.18 cubic feet per sack, and they used 5.23 gallons of mix water per sack. Allied pumped 91 barrels of cement slurry, 39.5 barrels of displacement, and there was 10 barrels of cement returns to surface. My inspection revealed no violations at this time. NOTE: Started pumping water on the 6H well and found the 2 inch ball valve on the conductor cap was split. Enviro Drill replaced the ball valve and Allied started pumping on the casing again. Had to shut down again because the weld on the conductor cap was leaking. They also found that the 5H and 10H conductor holes were communicating so they decided to pump this cement job first and then go back to the 5H and 10H conductor cement jobs. Region Supervisor Tom Hill was present for the cementing of the casing.
1393923913 2014-03-04 Drill / Deepen / Reopen No Violations Inspector James Bucksar and I were on site to witness the cementing of the 13 3/8 inch surface casing. Our inspection revealed that Nomac Drilling Rig # 71 has drilled a 17 1/2 inch hole to a total depth of 575 feet and ran 565 feet of 13 3/8 inch surface casing (54.5 pounds per foot / inside diameter is 12.615 inches). They installed 7 bowspring centralizers on the casing string on joints (2 on 1st, 2,3,6,9, and 12). Schlumberger cemented the casing with 514 sacks of Class A D901 with 1% D154, 0.5 B348, 0.15% B160A,, 0.2% D046, .125 pound per sack D130. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.219 gallons of mix water per sack. Schlumberger pumped 110 barrels of cement slurry, 80.5 barrels of displacement, and there was 21 barrels of cement returns to surface. They landed the plug with 275 PSI and bumped the pressure up to 728 PSI for the casing test. The casing test was approved, they released the pressure, the floats held, and there was 1/2 barrel of water flowed back when the pressure was released. Our inspection revealed no violations at this time.
1394076265 2014-03-05 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 inch intermediate casing. My inspection revealed that Nomac Drilling Rig #71 has drilled a 12 1/4 inch hole to a total depth of 1896 feet and ran 1888 feet of 9 5/8 inch intermediate casing (40 pounds per foot / inside diameter 8.830 inches). They installed 17 bowspring centralizers on the casing string; 2 on 1st joint,2,3,6,9,12,15,18,21,24,27,30,33,36,39,and 42. O-TEX cemented the casing with 690 sacks of Class H with 3% Gypsum, 2% CaCl, 0.8% MC500,0.5% C45, and 0.4% C-41P. The cement had a density of 15.8 pounds per gallon, a yield of 1.21 cubic feet per sack, and they used 5.14 gallons of mix water per sack. O-TEX pumped 149 barrels of cement slurry, 133 barrels of displacement, and there was 28 barrels of cement returns to surface. They bumped the plug with 1110 PSI and then took the pressure up to 1606 PSI and conducted the casing test. The casing test was approved, pressure was released, floats held, and there was 3/4 barrel of water flowed back. My inspection revealed no violations at this time.
1889442261 2017-03-02 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The #6H well is in production and is equipped with plunger lift assembly. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of eight 400 bbl production tanks, four labeled as containing produced water, and four labelled as containing condensate, and one low-pressure vertical separator. The tank battery is surrounded by a dike. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of:

6 producing wells 8- 400 bbl production tanks, four labelled as containing fresh water, and four labelled as containing condensate 6 sand separators 6 gas processing units 1 low pressure vertical separator located inside of the dike 1 vapor recovery unit 1 horizontal separator, 1 compressor station 2 bullet tanks Jersey barriers are installed around the production equipment. The access road and man doors were locked. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road was stable in in good repair.

2077328686 2018-11-19 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate and intermediate/production annular gauges displayed readings of zero psi. I performed a walk-around inspection of the pad and erosional controls appeared functioning. No violations were observed at the time of inspection.
