The well is equipped with a plunger lift assembly.  The production tanks are surrounded by a steel dike.  There was no standing water inside of the dike at the time of inspection.  The pad is surrounded by a fence and all man doors were locked.

| I received a call from DOGRM Emergency Operations and Response manager Scott King at 11:39 AM  on 04/05/2016 stating that Tom Cottis from the Carroll County EMA reported a fire on the Chesapeake P. Brown 9-13-5 Pad.  I arrived on site with DOGRM inspector Robert Roush at approximately 12:10 PM on 04/05/2016.  The fire was extinguished by the time we arrived on site. Fire departments from Fox Township, Loudon Township, and Carrollton Village were present on site, along with Tom Cottis from the Carroll County Emergency Management Agency, and several Chesapeake personnel.  We walked the site with Tom Van Der Muelen from Chesapeake. The fire occurred at the P. Brown #6H gas processing unit (GPU). The fire departments used fresh water to extinguish the fire and cool the unit.  Coordinates for the #6H gas processing unit where the fire occurred are 40.501740, -81.00191.  According to Chesapeake, a Chesapeake operator was performing routine maintenance when a leak in a stainless line in the GPU caused an audible hiss.  The operator opened the door to check the line, therefore introducing oxygen to the system and resulting in a flash fire.  The operator suffered burns and was transported to the hospital.  According to Chesapeake personnel, all of the six wells on location were shut in as a precaution.  Chesapeake does plan to re-start producing the unaffected five wells on location prior to remediation of the #6H damaged equipment.

| I was on location to conduct a preliminary restoration inspection.  The well is open flow with a wellhead Christmas tree assembly.  Jersey barriers were installed around the wellhead.  The well is properly identified with a clear, legibly-marked well identification sign.  The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment.  There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators.  There is a truck grounding area for trucks collecting condensate and/or produced water.  Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:

Jersey barriers are installed around the production equipment.  The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked.  Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation.  Drainage and erosion controls at the pad are in place and functioning.  A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure.  The pad has a crushed limestone surface, and is in good repair.  The lease road is stable and drivable. Preliminary restoration passed.
