At the time of inspection the site ID was installed. The erosion and sedimentation ESCGP was onsite. The PPC plan was onsite. The ERP was onsite. The well pad was covered by secondary containment and matting boards. No observation of accelerated erosion or sedimentation observed. The well was spudded. The cellar and conductor were installed. The Department recommends the Operator continue to monitor the site weekly and after measurable precipitation evets.

2608472 Primary Facility 2017-06-28 Routine/Complete Inspection No Violations Noted Well actively being drilled approximate depth is 12,800. Drill rig on containment. Containment appeared to be in sound condition.
2659984 Primary Facility 2017-11-14 Routine/Complete Inspection No Violations Noted At the time of inspection well drilling/production activity ongoing. Department observed the flaring of gas during inspection. All production equipment located on containment and appears to be structurally sound. No E&S issues.
2744878 Primary Facility 2018-06-19 Drilling/Alteration No Violations Noted Apex contracted Paterson rig 343 to complete well 5H. Well was drilled to 20,840 ft at 8 1/2". Production pipe will be set at 18,909 ft and cemented back. No issues at the site.
2787602 Primary Facility 2018-10-11 Routine/Complete Inspection No Violations Noted At the time of inspection well equipped for production. Three brine tanks within secondary containment and appears structurally sound. GPUs on secondary containment. No E&S issues.
