2582275 Primary Facility 2017-04-13 Routine/Complete Inspection No Violations Noted Routine inspection conducted at Anadarko's COP TRACT 289 Pad E on April 4, 2017. No one was on site during my inspection. There are three producing Marcellus wells on this pad. Natural gas leaving this pad enters the Pine Ck. No. 2 Water and Natural Gas Pipeline. Each well is properly identified with the well permit number and operator's contact information. Each well is connected to its own separate production unit. There are three production units on site. There are 4 (400)BBL produced fluids tanks located in secondary containment. There is approximately 2 inches of rain water within containment with a field screen reading of 82.0 uS/cm for conductivity. No issues observed with containment.

The original permit has been upgraded to an ESCGP2. The well pad remains its original size. No PCSMs have been installed to date. All E&S controls have been removed. I recommending installing the PSCM infiltration berms along the pad and access road per the drawings. No violations noted at this time.

2799444 Primary Facility 2018-11-05 Routine/Complete Inspection No Violations Noted On November 5, 2018, Department representative Brad Kline conducted a routine site inspection at ARD Operating , LLC. COP Tract 289 Pad E. Monthly tank maintenance log was onsite for review. No E&S issues were observed during the time of inspection. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover.

There are a total of 3 producing wells located on this well pad. All wells are tagged with API/well permit numbers. No bubbling within the cellars was observed. There are 3 GPU’s and 4 steel vertical production fluids tanks onsite. Both GPUs and production fluid tanks were stationed in secondary containment.

No violations noted.
