2424058 Primary Facility 2015-10-20 Preoperation Inspection No Violations Noted On 10/20/15, the Department was on site at the SWN RU 06 Flohs Well Pad to conduct a Preoperational Inspection. Inspection conducted with Rich Gulyas, SWN Construction Foreman, Seth Tratthen, SWN Regulatory Analyst, and Ryan Klemish, SWN Regulatory Analyst. The well permits that have been issued for this pad are the Flohs 4H (115-21846), 5H (115-21847), and the 6H (115-21848). All perimeter BMP’s in place to include red lined additions. Pad is 95% complete. Wash rack may be removed at this time since construction of site complete and temporarily stabilized. The first cross culvert on the access road is installed, but has not had the sump completed as per design because of a wood pile that is still present. This is not an issue at this time based on the size of the woodchip filter berm located down gradient of the discharge/log pile. Once the logs are removed, sump and level spreader will be installed. No violations noted.
2574594 Primary Facility 2017-03-21 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 12:45p.m. for a new spud inspection.

Minichi Contracting Group was on site to drill the hole and run the conductor pipe with a dual rotary rig. I spoke with driller Keith Bowman of Minichi, the rig was drilling the hole for the 6H and was at a depth of 41'. The projected depth for the conductor pipe is 88'. The drill cuttings are being put into a sealed role off container and hauled to the Keystone Landfill. I took a walk around the location and there were no obvious concerns noticed. I left the site at 13:10p.m.

2575450 Primary Facility 2017-03-21 Routine/Complete Inspection No Violations Noted I arrived on site to conduct a Routine Complete Inspection. At the time of the inspection, a spudder rig was active. The site is snow covered. I was unable to make an E&S evaluation due to the snow cover. Ryan Klemish (SWN) conducted the most recent inspection of the site on 3/13/17 and noted that no attention was needed.
2593827 Primary Facility 2017-05-15 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 9:40a.m.

I met with company man Robert Gray. W S G Rig #1 was on site and waiting the mandatory 8hrs on cement for the surface casing on the 5H. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection was 888' on the 5H. I left the site at 10:50a.m.

2625636 Primary Facility 2017-08-14 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 14:00p.m.

I met with company man John Woodburn. SDC Rig #45 was on site and drilling the lateral section of the well. The top-hole section of the well was drilled and completed on 5/13/17 by WSG Rig #1 to a depth of 1646'. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection was 10,117'. The proposed TD for the well is 11,913'. I left the site at 15:00p.m.

2661231 Primary Facility 2017-11-15 Routine/Complete Inspection No Violations Noted At time of inspection.

I met with company man Zack Evans. Halliburton was on site to frac the 4H, 5H and 6H wells. They were pressure testing the iron on the 4H when I arrived. There were 13 stages completed out of 34 stages on the 4H. The 5H was completed with 37 stages. The 6H was completed with 35 stages. The water was being pumped to the location from the Zeffer production water impoundment and the Odel fresh water impoundment. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2697752 Primary Facility 2018-02-13 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and was in production. The well tag was on the well and correct. There was containment around the well that made the inspection of the cellar limited. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. There were five empty 500bbl tanks still on the containment. I took a walk around the location and noticed two holes in the containment, there was a drip pan that was full of water and ice with oil on the top on the same containment. I contacted Frank Hoehle (WQS for the Department) and Steve Moyer of SWN to let them know of the holes in the containment.

2728819 Primary Facility 2018-05-11 Routine/Complete Inspection No Violations Noted I arrived on site at 1215 hrs on 4/30/2018 to investigate a self-reported release on the the well pad. That morning, Michael Spangenberg (DTE-Senior Environmental Engineer) called Briana Cunningham (WQS Supervisor, DEP) to self-report a methanol spill that had occurred on the site and originated from the WC 6 Flohs Pipeline, after a faulty methanol injection pump that had leaked an unknown amount of methanol onto the pad's surface. Present with me during the inspection was Gene Rickard (WQS, DEP) and Mike Spangenberg.

While conducting the inspection, Mr. Spangenberg told us that DTE had calculated that the total spill volume was no more than 11 gallons based on the methanol pump's flow rate and the amount of time that had passed since the pumps last inspection.

As of the time of the inspection, excavation had not yet begun, but the spill had been covered by a tarp to prevent further migration during precipitation. Mr. Spangenberg informed us that excavation was scheduled to occur at 0800 on 5/1/2018. Due t

2791745 Primary Facility 2018-10-19 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 2' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
