2493197 Primary Facility 2016-05-03 Preoperation Inspection No Violations Noted The Department was on site on 5/3/2016 to conduct a routine complete inspection for the JHHC well site (ESX29-115-15-0039).

There are 9 wells that will be associated with this site, and they are as follows: 115-22130 115-22131 115-22132 115-22133 115-22134 115-22135 115-22136 115-22137 115-22138 The site is under construction. All perimeter BMPs have been installed and there has been an additional filter sock added along the lower edge of the project. There are currently no issues with the site and this qualifies as a preoperational inspection.

2526055 Primary Facility 2016-09-22 Routine/Partial Inspection No Violations Noted
2550419 Primary Facility 2017-01-05 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 15:10p.m. for a new spud inspection, I met with company man Glenn Broussard. United Drilling was on site to drill the holes and run the conductor pipe. The 1H and 2H were drilled to TD at 92' and the conductor pipe was run in the hole. There was a rig on the 6H and the 10H, the rig on the 6H was running the conductor pipe and the rig on the 10H was at a depth of 44'. All the conductor pipe will be ran to 92' and cemented in. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 15:35p.m.

2555854 Primary Facility 2017-01-25 Follow-up Inspection No Violations Noted The Department was on site at the JHHC P1 Well Site located at 2295 E Rush Road Jessup Township, Susquehanna County to complete a Follow-Up Inspection to the Department’s 9/22/2016 inspection. During that inspection, the Department noted that the site did not appear to meet temporary stabilization requirements. During that inspection, the Department spoke with a representative of Barhite Excavating who explained that they were planning to deploy a method of stabilization later that day.

During this inspection, the Department reviewed the E&S Inspection Log and noted that the most recent inspection occurred on 1/25/17 and was conducted by Mike Brille (TEC). This inspection was noted to be a post precipitation event inspection. At the time of the inspection, it was noted that the site was covered in snow. Areas where the snow had melted were found to have grass growing. No obvious issues were discovered during this inspection.

2593830 Primary Facility 2017-05-15 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 12:30p.m.

I met with company man Joel Franks. Precision Rig 548 was on site and drilling the hole for the production casing. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection was 8318'. Projected TD for the well is 13,634'. I left the site at 13:25p.m.

2652899 Primary Facility 2017-10-26 Routine/Complete Inspection No Violations Noted At time of inspection.

I met with company men Jody Martin and Ken Wilson. Keane was on site to frac the 1H, 2H, 6H, and 10H wells. They just ran a plug and perf in the 2H well when I arrived on site. There were 13 stages completed out of 30 on the 1H. There were 56 stages completed out of 60 on the 2H. There were 23 stages completed out of 34 on the 6H. There were 31 stages completed out of 48 on the 10H. The water was being hauled to the site. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2654991 Primary Facility 2017-10-26 Incident- Response to Accident or Event No Violations Noted Arrived at the site at 12:19 in response to a reported spill of a residual lubricant to the JHHC well pad lease road, State Route 3027 and State Route 706. The weather conditions were cloudy skies and breezy with temperatures around 50 degrees. I was accompanied by Briana Cunningham, DEP O&G WQS during the inspection. We met with Ken Wilson, completions consultant of Cabot Oil and Gas on the lease road. Keane Frac is the subcontractor conducting hydraulic fracturing operations on the 2H well according to Mr. Wilson. Mr. Wilson provided the Department the following details in reference to the spill. "The spill occurred on the lease road. A wireline unit, #8015, was being sent back to New Stanton, PA to be serviced. A 3 ft x 3 ft pan (7.5) inches deep on a truck was completely full of a mixture of rainwater and vegetable oil, OCR782B biodegradable line spray, (53) gallons in capacity. We recovered (5) gallons in the pan leaving the spill volume at (48) gallons, with the majority of the impact occurring on SR
2684094 Primary Facility 2018-01-17 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The well tag was on the well and correct. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and venting to the same tank. There was a 500 bbl flow back tank on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2728564 Primary Facility 2018-05-10 Routine/Complete Inspection No Violations Noted I performed a RTNC inspection. The wells associated with this site are as follows: 115-22133, 115-22137, 115-22130, 115-22131. The wells were properly labeled.

The access road was solid and the pad surface was mostly solid as well. Due to recent rains there was some puddling on the pad surface, but no areas in danger of instability.

The grass was growing well and filling in on the cut and fill slopes. There were areas on the site which appeared to be recently touched up and mulched and these areas all appeared stable and growing.

There was a light/generator plant running near the entrance to the site. It was parked on containment however the right tire was on the berm of the containment rendering the containment ineffective. The plant was not leaking. It would be recommended the light plant be moved completely onto the containment to eliminate the possibility of a release occurring due to ineffective containment.

There were no erosional issues noted.

There were no violations noted.

2748531 Primary Facility 2018-06-19 Routine/Complete Inspection No Violations Noted The Department arrived on site at 1400 hrs to conduct a RTNC inspection. The Department was represented by Frank Hoehle (WQS-DEP) and myself.

The pad's access road and surface were solid and stable. The cut slope was solid and stable, with some isolated areas that are nearly bare and contain little vegetation. While the overall vegetative cover is still greater than 70 percent, these bare spots should be monitored by the operator to ensure 70 percent vegetative cover is maintained. The fill slope was well-vegetated with no apparent bare spots.

There were several areas of the pad's perimeter berm that had fairly large bare spots. Operator, please review these areas to determine if there is erodible material present in these areas. If there is, please determine the best method in your approved plan to achieve stabilization (Photos 8, 10, 11).

All the required PCSM BMPs were in place and in good condition, including ditches and rock aprons along the access road. There were several filter socks on site that w

2786536 Primary Facility 2018-10-10 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. There was water 7.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. The (20x13), (13x9) and (9x5) annuli vent pipes were connected together and going to the same poly vent tank. There was gas detected at the poly vent tank. There was a light plant being used as a generator at the entrance of the well pad on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
