2496236 Primary Facility 2016-06-27 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Gee 832 well pad (ESX29-117-15-0015) located in Middlebury Twp., Tioga Co. on 6/28/2016 @1300 hours. The weather was 80° partly cloudy with rain in the area. Three wells were observed on the pad surface. Two appeared to be in production. A small amount of fluid was observed in the containment surrounding the baffled tanks. The field conductivity was not tested due to a malfunctioning meter. The sediment trap located off of the NE corner of the pad was holding water at the time of the inspection. All E&S controls appeared to be stable and functional. No violations are noted at this time. I departed the site @ 1400 hours.

This inspection references the following well permits – Gee 832 1V 117-20562 SPUD 5/4/2010 RIS Gee 832 2H 117-21352 SPUD 11/2/11 Active Gee 832 2V 117-21538 SPUD 6/20/12 Active Gee 832E 22H 117-21731 Expired – not drilled Gee 832E 23H 117-21732 Expired – not drilled Gee 832E 24H 117-21733 Expired – not drilled
