2647219 Primary Facility 2017-10-11 Routine/Complete Inspection No Violations Noted On 10/11/17 at 1432 hrs I conducted a routine inspection of the Broughton well site. It was overcast, raining and 65 degrees F. No one was present during the inspection.

The 5H well was drilled in 2011 and recently stimulated from April - August 2017. The 3H well was spud on 12/7/15, but not drilled to total depth; a modification to the existing permit is currently under review.

At the time of my inspection, the site was idle. A liner containment structure was located along the west side of the pad. Three 500 barrel (bbl) tanks were staged in the containment near the southwest pad corner; I did not determine whether the tanks contained fluid. In addition, what appeared to be waste liner was wadded up and placed on the containment. The berm of the containment along much of the front side was removed. Standing water on the liner was field tested in several places for specific conductance; readings ranged between 35 - 56 micro Siemens/cm, which indicates that the standing water is from precipitation. This

2683736 Primary Facility 2018-01-11 Routine/Complete Inspection No Violations Noted I inspected the SWN Production Co Broughton Location located in Morris Twp., Tioga Co. on 1/11/2018 @ 1500 hours. SWN representative Tracy Gregory, Tom Chitwood Jr. and Seth Trathen were present during the inspection.

The Broughton Location is a well pad acquired from Chesapeake Appalachia LLC. The pad is full sized. SWN representatives stated due to safety concerns they will not reducing the pad size for restoration purposes. The 5H well (117-21141) was drilled in 2011 and stimulated between April and August of 2017. The 3H well (117-21797) was Spud 12/7/2015, but according to a previous inspection report, was not drilled to total depth. Two new permits were issued December of 2017, the 7H (117-21947), and the 8H (117-21948).

ESCGP-2 ESG29-117-17-0036 was issued 10/10/2017 to upgrade the existing PCSM BMPs. A bioretention basin has been constructed in the NE fill slope of the pad. The area appears stabilized, but it's not yet growing vegetation.

2708058 Primary Facility 2018-03-21 Site Restoration No Violations Noted The Department of Environmental Protection conducted an inspection of the SWN Production Company, LLC - Broughton TI-39 well pad located in Morris Township, Tioga County. On March 21, 2018 at 1045 hrs. The weather was 29°, some snow showers in the area, winds north at 10 mph.

The Department received a post-drilling well site restoration report (WSRR) for the TI – 39 well API#117–21797 3H on February 6, 2018. A landowner consent, authorizing the operator to not restore the land surface to approximate original contours until all the wells on the site have been properly plugged, provided The operator develops and implements a site restoration plan that complies with section 78a.65(a) and 78a.65(b)(2)-(7) and all PCSM requirements in chapter 102, was submitted with the WSRR.

No activity was observed on site during the inspection. Although greater than 70% vegetative cover was not observed during inspection, the site is stabilized. Minor issues were noted in the infiltration basin. Please see attached photos. N

2732726 Primary Facility 2018-05-22 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of the Broughton 3H was conducted May 22, 2018. At the time of inspection, the 3H well which is a conductor only, has been plugged and capped in an open cellar as of last week. The cap is labeled as a plugged well marker. No obvious issues were observed.
