Jerry McGinnis of Shell Appalachia (SWEPI) notified the phone duty officer that @ 2105 hours on 3/31/2016 that two barrels of drilling mud was released onto containment and ~5 gallons off containment, when a rubber hose malfunctioned. Present for the inspection were myself; Rory Hunter, Env. Coordinator (SWEPI); and Brett McCoy, OSR (SWEPI). The weather was 64°, partly sunny, winds 24 mph WSW. When the spill occurred SWEPI was in the process of transferring a synthetic based drilling mud from the tank to a truck via a pump. The hose from the pump to the truck ruptured, releasing 2 barrels to containment and ~5 gallons sprayed outside the containment. SWEPI contractor Haliburton cleaned up the mud immediately. 2522825 Primary Facility 2016-09-22 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the Gee 848 well pad located in Middlebury Twp., Tioga County on 9/22/2016 @ 1100 hours. A valid site ID sign was observed at the entrance to the access road. The E&S plan, well permit and onsite E&S inspection log were located in the mailbox. The site was last inspected by company personnel on 9/21/2016 with the comments “E&S inspection complete” SFB.

Present onsite for the inspection was Chris Underwood OSR, SWEPI. SWEPI is currently in the process of flowing back all three wells. The wells are flowing back into 1 open top tank/well so fluid production can be monitored. The fluid is then transferred to a battery of tanks located around the perimeter of the pad surface. Residual waste trucks are making periodic visits to empty fluid from the tank battery. The OSR was instructed to ensure fluid is removed from the secondary containment surrounding the tanks in a timely fashion.

2531030 Primary Facility 2016-10-24 Incident- Response to Accident or Event No Violations Noted The Department of Environmental Protection (DEP) inspected the Gee 848 well pad located in Middlebury Twp., Tioga County on 10/24/2016 @ 1100 hours in response to notification of secondary containment failure on 10/21/2016. The weather was 52° and cloudy.

A valid site ID sign was observed at the entrance to the access road. The E&S plan, well permit and onsite E&S inspection log were located in the mailbox. The site was last inspected by company personnel on 10/10/2016 with the comments “E&S inspection complete” SFB.

According to Chris Underwood, Completions OSR for SWEPI, on 10/21/2016 a higher than average precipitation volume was received by the area over the preceding few days. A large section of corrugated tubing along the backside of the tanks, used as the sidewall for the secondary containment surrounding the temporary produced fluids tanks, rolled out allowing the felt lining to slump and fluid to rush into the rock lined ditch behind the tanks. 2555942 Primary Facility 2017-01-27 Routine/Complete Inspection No Violations Noted The Department of environmental protection inspected the SWEPI LP Gee 848 wellsite located in Middleberry Township, Tioga County on January 27, 2017. The weather was 32° snow winds out of the West 12 mph. Three wells on the pad surface appeared to still be in the process of flowing back, but are in production. Company personnel were observed completing an E&S inspection during my inspection. All E&S controls appear to be functional. Fluid, not frozen, observed on the pad surface had a field measured conductivity of approximately 130 microsiemens/cm. No violations or issues were noted during this inspection. 2580337 Primary Facility 2017-04-07 Routine/Complete Inspection No Violations Noted The DEP inspected the SWEPI LP Gee 848 well site in Middlebury Twp., Tioga Co on 4/7/2017 @1230 hours. The weather was 36 degrees, freezing rain/snow mix. Three wells are observed in production onsite. Flowback equipment including sand traps, a gas buster and baffled baker tanks was observed. The production equipment appears to be operational. Standing water was observed on the pad surface and in the channels adjacent to the pad. E&S controls appear to be functional. Clean out of the roadside ditch along the access road will be necessary when weather allows. Please see attached pictures. No violations noted at this time. I departed the site at 1330 hours.

2719433 Primary Facility 2018-04-11 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted a routine inspection of the SWEPI LP Gee 848 well site located in Middlebury Twp, Tioga Co. on 4/11/2018 @ 1315 hours. The 22H, 23H, and 24H wells are in production. A sand trap and gas buster are still in place due to one of the Utica wells continuing to produce sand. A battery of rolloff tanks is still located onsite from completion of the wells. Fluid observed in the tanks secondary containment was screened for conductivity and found to be in the normal range. Puddles on the pad surface and in the rock lined ditches were screened for conductivity and found to be in normal range.

No violations are noted at this time. I departed the site at 1400 hours.

2745179 Primary Facility 2018-06-25 Routine/Partial Inspection No Violations Noted The DEP conducted a routine inspection of the SWEPI LP Gee 848 well site located in Middlebury Twp, Tioga Co. on 6/25/2018 @ 1210 hours. The weather was 69 degrees, sunny, 52% humidity. Winds NNW 12 mph.

JL Watts was present onsite reclaiming the pony pad after the P&A of the Gee 848 1V API#117-20662. SWEPI is in the process of completing interim restoration of the site. A small area around the perimeter of the main pad has been reclaimed. The fill slopes have been laid back and re-graded. The sediment traps appeared to have been converted into an infiltration basin. Please ensure all disturbed areas around the main pad are seeded and mulched as soon as possible.

The 22H, 23H, and 24H wells are in production. A sand trap and gas buster are still in place due to one of the Utica wells continuing to produce sand. Fluid observed in the gas buster secondary containment, was screened for conductivity, and found to be in the normal range.

2840684 Primary Facility 2019-02-15 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Gee 848 Well Pad in Middlebury Twp, Tioga County on 2/15/2019 at 1130 hours. The weather was cloudy and 39 degrees. The well pad is currently snow covered. The Act 9 sign and mailbox with the E&S Plan were observed at the site entrance. The onsite inspection log was in the mailbox by the production tanks.

There are currently 3 producing wells onsite (the 22H, 23h, & 24H). Crew was onsite performing service on one of the wellheads. All fluids field measured on pad surface were within normal conductivities. Containment structures were intact and functional. The reclaimed pony pad off the access road looked well vegetated and stabilized. No accelerated erosion was observed. The waterbars on the access road are intact and functioning.

No violations noted today. I departed the site at 1210 hours.
