On 5/1/18 when they ran in the hole and began drilling they were seeing uncommon and sporadic pressures. They shutdown and tried to troubleshoot the problem. Upon coming online with the mud pumps they noticed a loss of approximately 700 psi and shutdown again to trip out of the hole. Their mud motor had parted and left most of their BHA at the bottom of the hole. They latched on once while fishing and began to retrieve the BHA only to have it come loose and fall back to bottom. During current inspection it was believed to be that they had successfully retrieved most of the BHA from the well bore. Mr. Griffith still intended on running a trash basket and a magnet run to bottom to ensure all of the pieces that may have come loose while fishing are retrieved. Met with SWN representative Ed Blystone on location. There was a work over rig on location. Rig was in the process of running a retainer into the 5 1/2" production casing at 2967' in preparation to squeeze the perforations at 2991'. Halliburton was on location to perform the squeeze. Tested the 20" x 13 3/8" annulus and 13 3/8" x 9 5/8" annulus. 20" x 13 3/8" annulus had 100% LEL and 15% methane detected and 13 3/8" x 9 5/8" annulus had 100% LEL and 100% methane detected at time of inspection. Gas percentages were confirmed by Ed Blystone. Circulation was established at 2 bpm and 2100 psi. At time of departure Halliburton was in the process of spotting cement in the tubing to squeeze the perforations. Will follow up with Ed Blystone to confirm squeeze operations outcome.

A bioretention berm, along with a rock-lined ditch, provides PCSM. The site has been permitted to remain at full pad size. At this time, due the presence of a section of newly replanted vegetation which has not yet stabilized, as well as the presence of a frac tank, secondary containment, and other equipment not required for production, the site does not yet meet all of the post-drilling site restoration criteria under 78a.65.
