0102647935 2016-11-13 Annular Disposal No Violations I arrived at the well site and Jeff was blowing down the well. Jeff blew the well down to about 270 then built the pressure back up with the nitrogen to 320 on his gauge. I put my gauge in and opened the valve and the pressure was approximately 326 pounds of pressure per square inch. I had already looked at the UIC tab in RBDMS and the required test pressure was 300 pounds. I started the test at approximately 1:25 pm. I took a picture at 1:30 pm trying to capture the psi. because of my angle the picture looked a little shy of 326 psi. I took another picture at the completion of the test around 2:26 pm showing 324 psi. I told Jeff the test was succesful the well only lost 2 psi over the 60 min interval and that was less than 1 % so the well had passed.

0667336592 2018-04-20 Production Wells No Violations Mr. Jeff Burke asked me to come down and witness him running water into the annulus he uses for annular disposal. The well has been shut down for a couple days and he believes he can tell where the water table is based on how many barrels the annulus takes until it is full. The well has 4.5 inch production string and 640 feet of 7 inch casing. The annulus took 6.85 barrels to fill the annulus. 6.85 bbl. / .0208 bbl. per foot = 329.33 feet. This would estimate the water table for the annulus at 329.33 feet below the surface.

0758437970 2016-11-14 Annular Disposal No Violations On Monday while filling out the paperwork for the AD test I discovered several inconsistencies with the information in RBDMS. According to our records with the formula provided the well should have been tested at 327.12 psi. although the records indicated that at least 300 psi was required. I then called inspector Jon Scott to explain the situation. After talking with Jon and looking at the discrepancies he decided to call Columbus. Jon called me back and explained that the test would have to be performed again above 327.12 to be successful. I then called Jeff Burke and explained the situation. He said it would be a couple weeks before he could test again.

The well is equipped with a with a pumping unit, tubing and rods. Two 100 barrel steel tanks are located upslope from the well. A mechanical separator is located with in the tank dike. A 2 black plastic line is connected to a manifold with multiple valves, that connects to both tank drains. The black plastic line runs down the slope to a 1000 gallon poly tank located next to the wellhead. The poly tank drain is connected to the annulus.

I explained that I was not able to do it Monday because I had a meeting in Zanesville. I told him I would be able to do it on Sunday. He said that would work and we arranged to do the test at about 1pm.
