Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0196775824 2015-10-05 Production Wells No Violations Darrel Porter and Contractor Andy Browning took equipment in to the tank site to rebuild the dike. They were going to remove the plastic tank that was half full of sludge. Darrel claims that he called Mike Corp and there was no answer. They would come back to remove the tank after Corp cleans the tank out. They were headed to Licking county.
0456240756 2004-10-18 Production Wells No Violations Tanks?
0610328346 2008-11-26 Production Wells No Violations Producing. No problems noted. Tanks?
0670397157 2014-10-10 Administrative Inspection No Violations Daryle Porter called and said that Ted Denardo of the Shelly company thinks that there is a stone pillar under the tank. Darlye Porter of Porter Petroluem will have a wait and see approach. Porter will leave the tank where it is. He will keep the tank at a minimal level. The Shelly Company will reinforce the subsidence with bolts and a plate.
0932380067 2014-10-01 Production Wells No Violations The New Philly office called on a well that had a tank battery that was close to the subsidence on a mine at the west end of the Chesterhill gravel quarry. Producing well with a pumping unit and the rod and tubing annulus are both open. The tank I could not find. I will stop by the mine office in the morning.
1316423592 2014-10-02 Production Wells No Violations I stopped by the Shelly office and met withTed Denardo the Under ground mine manager. He and I went up to the tank battery for the well. There is a centerline of subsidence marker at coordinate 39.84511,-82.13988. The second marker is at 39.84497,-82.13974. The tank battery is at 40 away. I set down with the company and the decision was made to relocate the oil tank and equipment.
