1280296169 2017-08-30 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1847. The lease road and pad surface was composed of crushed gravel and remained in good repair. Correct identification was posted at the entrance and well head. A locked chain link fence remained around the pad. The well head was protected by jersey barriers. The cellar was backfilled with gravel. The annular gauges were plumbed above grade and identified. The 9.625 gauge indicated 0psi. The 5.5 annulus gauge identified 0 psi. The 5.5 production casing was equipped with tubing and was in production. No leaks were observed. Production equipment on the pad consisted of one (1) sand knock out and GPU, one (1) horizontal dehydrator, one (1) hydrate inhibitor tank, one (1) thermal generator, one (1) meter run and three (3) 400 bbl production fluid tanks in containment. The containment was lined with impervious material and was holding water with no sheen. Valves were closed and plugged. No violations were observed during this inspection.

They are going to change from air drilling to fluid. Permit and conditions are posted in the company house. There were no noted violations at this time.

1976196785 2014-10-24 Preliminary Restoration No Violations I was on site to assess the completion of preliminary restoration. There is a sign board at the access road entry featuring the well name, permit number and emergency contact information. Identification is also posted at the wellhead and tank battery. The access road has a padlocked gate near Canal Road (State Route 9). The access road is in good repair, and drainage and erosion controls are in place. Ground cover is well established along the access road. I found the production pad to be enclosed entirely with a chain link fence with barbed wire at the top, and with a padlocked gate. The fence is in good repair. The pad has a crushed limestone surface, and is in good repair. The well is not equipped with any production equipment. There are concrete barriers protecting the well from vehicles. The tank battery is comprised of six 400 bbl storage tanks. The tanks are located within a spray-lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Along the interior of the chain link fence there is a site drain installed. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Well pad was housing twenty-two 500 bbl frac tanks. No production equipment is yet in place. Preliminary restoration approval will not be granted pending installation of production equipment and re-inspection.
2014995687 2014-06-12 Drill / Deepen / Reopen No Violations Nomac Rig 70 drilled a 8.750 hole to a total depth of 20540. They ran 20517 of P110HC 20# 5.5 production casing. Nomac Rig 70 circulated the hole after landing the production casing. O-Tex Pumping 50 barrels of Barite followed by 660 sacks of Lead 50/50 POZ 10% NaCl (BWOW), 0.6% C41P 11.5 # cement. This was followed by 2750 sacks of Premium H 5% KCI (BWOW), 0.6% C-12, 0.4% C-51, 0.1% C-5, 0.05% C-2015.3# Tail cement. Displaced with 450 barrels of fresh water. Floats held 3.5 barrels back in the in tub. Job completed. I only witness the lead cement.
2135620601 2014-05-24 Drill / Deepen / Reopen No Violations Nomac Rig 70 drilled a 17.5 hole 559.56 and ran 556 of 13 3/8 J-55 surface casing. O-Tex Pumping was on location to cement the surface casing. After the safety meeting they pressured tested there lines. They pumped 30 barrels of gel, followed by 10 barrels of fresh water, followed by 109 barrels of 15.6# weight of Premium H 2% CaCl2, o.4%1P, 1/4lb/sk Cello Flake. Displaced with 65 barrels of fresh water. Plug landed and held. There was 20 barrels of cement to surface.
