0017171163 2013-10-21 UrbanProduction Wells No Violations The producing unit consists of a mechanical plunger lift unit and above-ground storage tank (AST) battery. The battery is comprised of 1 210-barrel steel production tank and 2 separator units. The battery is shared with the #262 unit. The wellhead is partially enclosed in a chain-link fence. There is some leakage of gas occurring from around the 4-inch to production swage. The battery is not enclosed. The ID is missing and the 210 production tank is completely rusted through and not fit for use. I have contacted Broad Street and advised about the issue.
0762457904 2019-02-07 UrbanProduction Wells No Violations I conducted a compliance notice inspection for compliance notice 1546876028, regarding the installation of identification at the well and tank battery locations. At the time of this inspection, no identification has been installed at the well or updated to reflect the current owner information at the tank battery. No other changes since my last inspection have occurred at the well site location. I contacted well tender T.J. McCumber from M&R Investments Ohio, LLC regarding the identification needing updated and installed. Mr. McCumber stated that he would update and install the necessary identification. The current plugging order on the well expires on 7/6/2019. To date, the well has not been plugged.
1546876028 2019-01-07 UrbanProduction Wells Violations Noted I conducted an inspection on this well site as a request from landowner Jim Parks. Mr. Parks is looking to sell the property and was inquiring on the status of the well. At the time of this inspection, the well was equipped with a plunger lift assembly and not in production. The gate valve on the tubing and the valve on the flow line were both closed. There was a domestic line plumbed off the 4.5 inch casing with a regulator and closed valve. Mr. Parks stated that the well volume was not sufficient to utilize for domestic use. Production records submitted in 2015 was zero, 2016 was 10 mcf of gas and in 2017, production was sumbitted as zero. The tank battery was comprised of one 210 BBL tank and two mechanical separators. Valves on the separators were closed on the inlet sides and the charts on the sales lines were dated 5/14/14-5/21/14 with Beldon and Blake company name on them. The 210 BBL tank had small holes near the top of the tank, but no leaks were observed. The diked tank battery was shared with well 2-0262. Identification located at the tank battery had current owner information but was missing the permit numbers and no identification was located at the well. Lease access is the landowner driveway and stable. I contacted M&R Investments Ohio, LLC and spoke with field supervisor Bill Fitzmartin on the status of the well and also the need for identification at the well and tank battery. Mr. Fitzmartin stated that he would have identification installed at the well and tank battery. Mr. Fitzmartin also stated that the well status for future production was unknown at this time.

The well owner shall submit a written plugging schedule to Inspector Shannon Mullins at ODNR Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685 within 30 days. The well owner/operator will be notified if the plugging schedule is approved or denied. The Clapacs #1 shall be plugged within 180 days. In the interim, the owner
