1416601318 2014-11-20 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. The well and battery are located at the terminus of a lengthy, dedicated access road which enters off of Main St. in Perry. The wellhead pump jack and battery are in good condition. The pumping unit is temporarily offline while repairs are made to the electric service drop which was severed by a falling tree. The tank battery is comprised of 1 100-barrel production tank and 1 100-barrel brine tank, with separator unit. The wellhead and battery are in separate chain-link enclosures. Secondary containment is intact. Enclosure gates are locked. I did not observe any signs of leakage or problems during this inspection.
1690760250 2017-06-08 Production Wells No Violations I inspected this well as part of routine inspection activity. The well with pump-jack system and associated tank battery located on a larger, otherise undeveloped parcel, most of which consisted of woods and meadow with mature, wild vegetation. The tank battery is shared with the Middletown Bank #2 (#2-0558). The fittings and connecitons at the wellhead and battery were secure. I walked the areas around the well and battery and found so indications or signs of problems. I noted that the signage had become faded and notified Ron Gibson about the need for maintenance. Production reports were current and there were no indications of deficiencies with bonding or insurance in a review of electronic file information conducted on this date.
